Frailty in Canada — Dr. Kenneth Rockwood

Frailty in Canada — Dr. Kenneth Rockwood
As Canada’s population ages, it is expected that an increasing number of people will become frail. Individuals living with frailty have an increased risk of the following: hospitalizations, longer hospital stays, hospital readmissions, emergency department visits and in-hospital death. On this episode of the Canadian Health Information Podcast, host Avis Favaro speaks with Dr. Kenneth Rockwood, clinical research professor of frailty and aging at Dalhousie University in Halifax, about the harms of frailty and what we can do to better protect older adults at risk. Ian Powell: Health Commentator on St John pulling ambulances off the road and the underfunding of the health system

Ian Powell: Health Commentator on St John pulling ambulances off the road and the underfunding of the health system

Lack of resources are being blamed for St John's move to pull some ambulances off the road.

The service is aiming to rein in costs for when a worker is sick or goes on leave.

Health commentator Ian Powell told Mike Hosking that it comes down to having enough staff, and funding isn't matching the increase in health demand.

He says this is not just a St John's problem there's a problem in the model, and there's a problem tangled up in the generalised under-funding of our health system.

Kerre Woodham: A breath of fresh air for our health system

Kerre Woodham: A breath of fresh air for our health system

As Mike Hosking and I exchanged morning pleasantries today, I said to him “Wasn't Dr Shane Reti a breath of fresh air?”

“Breath of fresh air?” said Mike, “He was a howling Nor Westerly of fresh air!”

And by crikey, he was. Even those who didn't vote National must surely appreciate that here is a knowledgeable, passionate man in charge of his portfolio. He is under no illusions that New Zealand's health system is in dire straits. But he also understands that the system is absolutely underpinned by he tangata, he tangata, he tangata - the people, the people, the people.

Compare that with one of the former health ministers, Andrew Little, who just flatly refused to concede that the health system was in crisis.

The most he could manage to choke out was that we had a health system that was under major pressure and had major challenges. Initially, he didn't put nurses or midwives on the fast-track residency pathway list, kept saying we've got hordes of nurses lined up to be with us.

    Hordes of them.

That's just simply not true. The New Zealand Nurse Organisation said it was flabbergasted nurses weren't included on that list, and he took a swipe at the Nurses Organisation. You know, the very people he's supposed to be working for and with.

Of course, as previous health ministers have found, words are easy, it is actions that count. And while it's fantastic that Doctor Reti has the safety of ED staff top of mind, how he will ensure that safety is another matter, security guards have limited powers. To really protect staff we'd need a police officer or two in every ED.

Still, when you see the number who were babysitting the gangsters on their way to Bird's funeral in Foxton, we clearly have officers available. It's just where their bosses choose to deploy them.

But to get back to the positives, it's the messaging I'm loving. You know, Dr Reti said it's broken. We've got a health system that's broken. He doesn't want any more seismic upheavals, he said the staff are exhausted, they've gone through Covid massive changes, a government that was on the back foot when it came to recruiting from overseas. Any more massive changes and I imagine that will be the straw that breaks the camel's back.

You also get the fabulous message that you will not treat people who want to help you with disrespect and violence. It is not acceptable to do so. The honesty and the authenticity in Dr Reti’s interview was a signal to us all that henceforth, basic decency is expected of members of the community. And there will be consequences if you cannot dredge up that decency from deep within you.

    Amen. Doctor Reti. Amen.

Tim Beveridge: How do we prevent ourselves from relying on the health system?

Tim Beveridge: How do we prevent ourselves from relying on the health system?

I’ve gotta be honest with you that I’m feeling a little punch drunk digesting the latest stories and statistics about our health system under pressure.

So, bear with me while I throw a few stats at you, just to package it all up.

Hospital admissions for babies and pre-schoolers is up 35%, and those are the admissions that could have been prevented with better primary care. Up 35% in the year ending June. It’s even worse in Auckland, the stat is 66%.

The number of patients waiting more than four months to see a specialist has jumped 46% in the last year.

And you might even recall the recent story that half of the number of Kiwis needing heart surgery waited longer than the maximum time frame which is considered appropriate by their specialists. Half of them.

I’m not sure which of the figures concerns me the most, but perhaps it is the one that I began with, that the hospitalizations of young children which could have been prevented had they simply been able to see a doctor in a timely fashion.

I have seen comments that there are problems that Te Whatu Ora, and that health care specialists feel that the ministry is disconnected from the pressure that they are under.

It’s difficult to see if there could be any other issue facing the incoming government that could be of a higher priority than getting on top of the challenges that our health system faces.

Seriously. Can you think of a higher priority issue than health?

But that’s not going to happen overnight, and it might sound a touch paranoid but as a parent I have found myself saying to my kids “don’t do that unless you want to go and spend a day sitting in A&E.”

I think the thing these stats do is place the focus back on us, and how we look after ourselves.

I wonder how many of us are still keeping up with private health insurance, no matter the increase in the cost of it. How many of us are thinking: “I’d better hang onto that because I might need it.”

When it has to be obvious that the challenge to all of us is how we look after ourselves in the interim, how do we prevent ourselves from having to rely on the health system?

Because regardless of when or how you get your care it doesn’t seem like it’s likely to happen —if you rely on the public health system— in the time frame is going to enable you to get back to living your life as quickly as possible.

The new Government, they’re going to introduce targets which is all very well but how are they going to meet them?

I don’t mind saying that, in the back of my mind, I probably am taking slightly better care of myself simply because often a few simple choices can make the difference between keeping on a healthy track and going down the line of sitting in a queue for health care that isn’t coming too soon.

Because the only certain thing we have control over is the ability to look after ourselves. Thats surely the first port of call.

    Thats where I am at.

Look the headlines are disturbing, if you group them all together you’d just be living in a state of worry all the time.

And I know there are a lot of people getting great care with our wonderful health workforce, and deep down I still hope that the care will be there when I need it, but I’m certainly not counting on any longer in the way that I used to.
