    Let us tell you a bit more…  This year, 2023, marks fifty years since Os Guinness published his first book back in 1973, “The Dust of Death.” It started what has become a prolific writing career for the well-known social critic. Today in our time with Os, we hope to introduce (or reintroduce) you to Os Guinness, and his profound contributions to discussions about freedom, culture, faith, and the quest for meaning and purpose.

    Stephanie and Os start by discussing Os’ latest book, a truly brilliant, magnificent little gem for all of us to be inspired by, and titled, Signals of Transcendence.

    The modern world is a place of great distraction, and it can be difficult to make sense of our human existence. But at some point in our lives, we may experience particular moments that prompt us to search for something deeper. Sociologist Peter Berger described these hints and clues as “signals of transcendence” that awaken us to unseen realities. In Signals of Transcendence: Listening to the Promptings of Life, Os Guinness tells stories of people who experienced signals of transcendence and followed them to find new meaning and purpose in life. Notable figures such as Leo Tolstoy and C. S. Lewis as well as lesser-known individuals experienced a variety of promptings that signaled to them that life could not continue as they had thought. Through unsatisfied longings or disillusionments or glimpses of beauty or joy, these moments drew people toward epiphanies of transformation. And the same can be true for us, should we have the courage to follow the signals wherever they may lead.

    BONUS! READ A SAMPLE OF SIGNALS OF TRANSCENDENCE

    “I’m at a point in my life where I realize that there has to be more to life. Something must be missing.” This remark, made to me by a business leader in Silicon Valley, expresses what countless people come to see in their own way and say in their own words. Previously, they were mostly contented in some season of life; some were wealthy, successful, and even highly celebrated in one field or another. But they reached a point where they knew in their heart of hearts that none of it quite satisfied as they hoped. Who am I? Why am I here? What is life all about? Life raises such questions to all of us at some point, and certain experiences break into our lives that spur us to question whether our answers are deep enough, prompting a search for what we sense is missing—an unnamable something more. Life itself is extraordinary, and somehow, we all want to know what it is to live a worthy life, one that fulfills the promise of life. Peter Berger, the eminent social scientist, described the experiences that trigger such longings as “signals of transcendence”— arresting and intriguing experiences that both capture our attention and call for further explanation. The thrust of these signals points to some meaning beyond themselves, and they won’t let us off the hook until we stir ourselves to find what it is. Such experiences puncture one’s satisfaction with the status quo and push one to search for something beyond. The signals stir in us a sense that there must be something more to life, but what is that often unnamable something? In stirring us, signals of transcendence are a prompting by life itself, as it were. They trigger both a contradiction and a desire, and call into question the past, the present, and the future. They challenge the present and the past by contradicting the temptation to settle down and be satisfied. They challenge the future by spurring a desire to search for the something that is missing, that toward which the experience is hinting. In so doing, the signals lay bare some aspect of our human existence that we have forgotten or suppressed, at least partially— including things lost and left behind. Such aspects of a fuller and more complete reality must be rediscovered if life is to be lived to the full. Equally, an understanding of those aspects has to be grounded solidly if it is to be truly fulfilling. Hence the quest for faith and meaning triggered by the signals—the quest for meaning that is adequate and faith that is true. Follow the signals and discover more of the reality of who we are and what the universe and life are about; then our lives will be better aligned and more able to be free and fulfilled. Freedom, after all, is simply the ability to be who we are, to think freely, to speak freely, and to act freely. But who in truth are we, why are we here, and what is life about? The signal is power packed with the thrust of such questions.”

    MEET OS GUINNESS

    OS GUINNESS (DPhil, Oxford) has had a lifelong passion to make sense of our extraordinary modern world and to stand between the worlds of scholarship and ordinary life, helping each to understand the other—particularly when advanced modern life touches on the profound issues of faith.

    As a frequent speaker and prominent social critic Guinness has addressed audiences worldwide, from the British House of Commons to the US Congress to the St. Petersburg Parliament. He is a senior fellow at the Oxford Centre for Christian Apologetics and was the founder of the Trinity Forum.

    Born in China to missionary parents, Guinness is the great-great-great-grandson of Arthur Guinness, the Dublin brewer. After witnessing the climax of the Chinese revolution in 1949, he was expelled with many other foreigners in 1951 and returned to England where he was educated and served as a freelance reporter with the BBC. Since coming to the United States in 1984, he has been a guest scholar at the Woodrow Wilson Center for International Studies and a visiting fellow at the Brookings Institution. He was the lead drafter of the Williamsburg Charter, celebrating the First Amendment, and has also been senior fellow at the EastWest Institute in New York, where he drafted the Charter for Religious Freedom. He also coauthored the public-school curriculum Living with Our Deepest Differences.

    Guinness is the author or editor of more than thirty books, including The Call, Time for Truth, Unspeakable, The Magna Carta of Humanity, The Great Quest, Zero Hour America, and others.

    Stephanie welcomes spiritual director, university professor and author James Bryan Smith to ponder the weight of God's love for us as His chosen creation. In his latest book, "The good and beautiful you" Jim invites us to peel back the layers of many lies we have commonly been fed in the church--about our worthlessness and predicament when our identity is primarily defined as "sinner." Jim likes to remind everyone - inlcuding himself - that Genesis 1 and 2 precede Genesis 3, and that grace and love precede sin and the fall! They discuss the difference between soul and self, and why the soul has such demanding needs. James tells us now the needs of our soul can be met in Christ alone.

    Jim describes ten things our soul cannot endure, and ten God-created longings. He says, "the soul is made to soar in trasncendance." Yet this is the furthest thing from self-help and narcissism! He invites us to "put to death our need ot establish our worth through what we accomplish." Who does not need to hear this in our generation of ever-hurried people? 

    He talks of the sacredness of our God-bestowed body: "we cannot engage in a relationship with God apart from our bodies." Jim also talks about our deep-seated need to be wanted and desired. He says, "our identity is given by God - and it will find us! We don't have to search for significance; in Christ significance has found us." He prays and invites us to join him in asking the Lord, "O God help me believe the truth about myself, no matter how beautiful." Again, this is the furthest thing from narcissisim because it invites us to then consider every one around us in the same light - considering others as more important than ourselves, imitating Christ Jesus as Paul challenges us in Philippians 2.

    Jim says, "I am my own worst critic. I live under the tyranny of the ideal self." Anyone else can use a dose of truth against self-criticism? If God does not spend His days thinking negative thoughts about us, then why should we think those thoughts about ourselves - or others?

    Os Guinness is an author and social critic. Great-great-great grandson of Arthur Guinness, the Dublin brewer, he was born in China in World War Two where his parents were medical missionaries. A witness to the climax of the Chinese revolution in 1949, he was expelled with many other foreigners in 1951 and returned to Europe where he was educated in England. He completed his undergraduate degree at the University of London and his D.Phil in the social sciences from Oriel College, Oxford. Os has written or edited more than thirty books, including The Call, Time for Truth, Unspeakable, A Free People’s Suicide, The Global Public Square, Last Call for Liberty, and Carpe Diem Redeemed. His latest book, The Magna Carta of Humanity: Sinai's Revolutionary Faith and the Future of Freedom, was published in 2018. Since moving to the United States in 1984, Os has been a Guest Scholar at the Woodrow Wilson Center for International Studies, a Guest Scholar and Visiting Fellow at the Brookings Institution, and Senior Fellow at the Trinity Forum and the EastWest Institute in New York. He was the lead drafter of the Williamsburg Charter in 1988, a celebration of the bicentennial of the US Constitution, and later of “The Global Charter of Conscience,” which was published at the European Union Parliament in 2012. Os has spoken at many of the world’s major universities, and spoken widely to political and business conferences across the world. He lives with his wife Jenny in the Washington DC area.

    Can we find true freedom for both society and the human soul? | with Os Guinness - Part 1

    Episode 114 - Stephanie is deeply passionate about the message of Os Guinness. How are we to understand the times we are living in? How are we to live as Christians and agents of both reconciliation and truth in a culture that seems to seek neither? How are we to fulfill our calling, individually and corporately? The stakes could not be higher--and the answers are found in Scripture. Join us for a fascinating conversation, a deep dive into the human heart, and an ambitious call to live for such a time as this.

    In these stormy times, loud voices from all fronts call for revolution and change. But what kind of revolution brings true freedom to both society and the human soul? In his latest book and on Gospel Spice today, cultural observer Os Guinness explores the nature of revolutionary faith, contrasting between secular revolutions such as the French Revolution and the faith-led revolution of ancient Israel.

    He argues that the story of Exodus is the highest, richest, and deepest vision for freedom in human history. It serves as the master story of human freedom and provides the greatest sustained critique of the abuse of power. His contrast between “Paris” and “Sinai” offers a framework for discerning between two kinds of revolution and their different views of human nature, equality, and liberty. Drawing on the Hebrew and Christian Scriptures, Guinness develops Exodus as the Magna Carta of humanity, with a constructive vision of a morally responsible society of independent free people who are covenanted to each other and to justice, peace, stability, and the common good of the community. This is the model from the past that charts our path to the future.

    MEET OS GUINNESS

    Os Guinness is an author and social critic. Great-great-great grandson of Arthur Guinness, the Dublin brewer, he was born in China in World War Two where his parents were medical missionaries. A witness to the climax of the Chinese revolution in 1949, he was expelled with many other foreigners in 1951 and returned to Europe where he was educated in England. He completed his undergraduate degree at the University of London and his D.Phil in the social sciences from Oriel College, Oxford. Os has written or edited more than thirty books, including The Call, Time for Truth, Unspeakable, A Free People’s Suicide, The Global Public Square, Last Call for Liberty, and Carpe Diem Redeemed. His latest book, The Magna Carta of Humanity: Sinai's Revolutionary Faith and the Future of Freedom, was published in 2018. Since moving to the United States in 1984, Os has been a Guest Scholar at the Woodrow Wilson Center for International Studies, a Guest Scholar and Visiting Fellow at the Brookings Institution, and Senior Fellow at the Trinity Forum and the EastWest Institute in New York. He was the lead drafter of the Williamsburg Charter in 1988, a celebration of the bicentennial of the US Constitution, and later of “The Global Charter of Conscience,” which was published at the European Union Parliament in 2012. Os has spoken at many of the world’s major universities, and spoken widely to political and business conferences across the world. He lives with his wife Jenny in the Washington DC area.

    Como regla general, los vasopresores se administran mediante infusión continua. Inclusive, algunos vasopresores idealmente deben ser administrados mediante línea central (aunque hoy día hay cada vez más evidencia que se pueden comenzar por líneas periféricas temporeramente). Sin embargo, en ciertas circunstancias muy particulares, la administración de una dosis en bolo (IVP, por sus siglas en inglés) puede ser útil y efectiva.

    La diferencia entre una droga y un veneno es la dosis. - Paracelso

    Caso

    Usted está atendiendo a una paciente con fiebre alta, tos productiva y dificultad para respirar. Los signos vitales son pulso 122, respiración 28, presión arterial 69/28, SpO2 82%. El estado mental del paciente está deteriorándose rápidamente. Usted decide intubar la paciente y comienza a preoxigenar lo más que puede antes de realizar la laringoscopía.

    Crónica de una muerte anunciada

    Si el caso anterior no le da escalofríos, probablemente no entiende la gravedad de la situación. El caso anterior describe a una paciente en shock séptico, que necesita desesperadamente de fluídos IV, vasopresores y ventilación mecánica. Si no recibe las tres, muy probablemente va a fallecer rápidamente. El problema es que también es muy probable que fallezca por el intento de realizar cualquiera de esos mismos tratamientos si no se coordinan apropiadamente.

    Los fluídos y vasopresores van a hacer su efecto, pero no va a ser de inmediato.

    Polinización cruzada

    Los anestesiólogos han utilizado la epinefrina y fenilefrina en dosis bien bajas para contrarestar los efectos temporeros de los agentes de inducción en pacientes que están en peligro inminente de un colapso cardiovascular. No es un concepto nuevo. Como si se tratara del polen de una flor que se transporta hacia otra flor, uno de los nuevos temas que ha causado furor en los foros de medicina de emergencia lo ha sido el uso de estos dos agentes para contrarestar periodos temporeros de hipotensión.

    Bolo vs infusión

    Para propósitos de comparar la lógica de usar un medicamento en bolo vs en infusión, veamos el ejemplo de la sedación de un paciente intubado. Supongamos que usted está atendiendo a un paciente intubado y se le acabó la sedación. De repente su paciente está despierto y moviéndose. Se mueve tanto que usted cree que podría extubarse por accidente. Usted decide administrar un bolo del agente de sedación para lograr el efecto inmediato y luego procede a reiniciar la infusión de mantenimiento.

    De la misma manera que usted puede considerar un bolo inmediato y luego una infusión de mantenimiento para un agente de sedación, usted puede considerar un bolo inmediato de un vasopresor para lograr el efecto deseado y luego mantener una infusión.

    Como en todo, hay riesgos a considerar

    La administración de cualquier medicamento vasoactivo conlleva riesgos. En ningún orden de severidad o importancia, algunos de los riesgos pueden ser la extravasación, hipertensión, taquicardias y/o arritmias, por nombrar algunos.

    En otro episodio hablaremos sobre cómo manejar estos riesgos, especialmente el de la extravasación.

    Pero, lo que sí quiero dejar claro es que usted debe administrar este tipo de medicamento solo en pacientes que usted pueda monitorear en tiempo real de forma constante. Por ejemplo, la epinefrina actúa mucho más rápido que la dopamina, por lo que usted debe tener la habilidad de poder evaluar casi en tiempo real el movimiento de la presión arterial y el pulso para hacer los ajustes correspondientes.

    Habiendo dicho esto, las dosis que se mencionan a continuación son lo suficientemente bajas como para que no ocurran cambios demasiado dramáticos. No obstante, es importante preveer.

    Indicaciones muy específicas

    La dosis IVP de epinefrina debe considerarse exclusivamente en pacientes que tienen un periodo de hipotensión temporero. El efecto de la dosis IVP será solo de menos de 10 minutos. El objetivo de la dosis IVP no es mantener la presión sanguínea por largo tiempo, sino mitigar el efecto de un procedimiento que pueda provocar hipotensión transitoria.

    En algunos casos de hipotensión severa donde el paciente súbitamente comienza a empeorar dramáticamente, la dosis IVP puede servir de puente para lograr un efecto inicial inmediato en lo que la infusión (y líquidos, de ser necesarios) hace su efecto.

    Epinefrina o fenilefrina

    La fenilefrina es exclusivamente alfa-agonista, mientras que la epinefrina es alfa- y beta-agonista. Dependiendo de la condición, quizás uno sea más conveniente que el otro. Sin embargo, la disponibilidad de fenilefrina podría ser muy limitada en muchos lugares, por lo que la epinefrina se convierte de facto en el medicamento a elegir.
