    Summary:

    • Serious bacterial infections (SBIs); which include UTI, meningitis, and bacteraemia, lead to dangerous complications in infants.
    • This was a prospective observational study between 2011 and 2013, looking at previously healthy febrile infants aged 60 days or younger, who were evaluated for SBIs.
    • The clinical rule considered the urinalysis, absolute neutrophil count (ANC), and serum procalcitonin, and had a sensitivity of 97.7%, with a specificity of 60%, and a negative predictive value of 99.6%.
    • One infant with bacteraemia and two with UTI who were misclassified. 
    • This tool decreased the number of lumbar punctures performed, antibiotics administered, and infants admitted to the hospital. 

    Take-Home Points:

    • Remember that these infants are the most at-risk population; do more rather than less.
    • Safety net: no matter what you do or what investigations reveal, always have a safety net!

    Pan, P. (2020). Validation of the Testicular Workup for Ischemia and Suspected Torsion (TWIST) Score in the Diagnosis of Testicular Torsion in Children With Acute Scrotum. Indian Pediatrics, 57(10), 926-928. https://doi.org/10.1007/s13312-020-1992-6.

    Summary:

    • The TWIST score includes - testicular swelling (2), hard testicle (2), absent cremasteric reflex (1), nausea or vomiting (1), a high-riding testicle (1). 
    • Low risk was a score of 0-2, intermediate risk was a score of 3-4 and high risk was a score 5-7.
    • Of those with testicular torsion, the mean TWIST score was 5.7 (none in low-risk category, 13 in intermediate-risk group, and 55 in high-risk group). 
    • Of those without testicular torsion, the mean TWIST score was 1.46 (21 in low-risk group, 7 in the intermediate-risk group, and 0 in the high-risk group).
    • All patients with a high-riding testis or absent cremasteric reflex were found to have testicular torsion.


    Take-Home Points:

    • This score reminds us what we should be looking for when assessing a patient with acute testicular pain and thus help to build clinical gestalt, however, there is not enough evidence at present to rely on this tool alone (although this may change in the years to come). 
    • It is important to involve the urologist/general surgeon if there is any suspicion of torsion.
    • Given the time-critical nature of this presentation, patients should be taken to the theatre (rather than further investigated with ultrasound) if testicular torsion is suspected.

Salbutamol and ipratropium by inhaler is superior to nebulizer in children with severe acute asthma exacerbation: Randomized clinical trial.     Take-Home Points:

    • 103 children between the age of 2 and 14 years with severe asthma exacerbations (pulmonary score 7) were randomly allocated to a nebulizer or metred dose inhaler (MDI) and spacer with nasal prong oxygen.
    • The primary outcome was the rate of hospitalization with the secondary outcome being oxygen saturations at 60 and 90 minutes.
    • Children in the MDI group had significantly improved oxygen saturation from 60 minutes compared with the nebulizer group, with significantly lower rates of admission to hospital (5.8% vs. 27.5%). 

    Summary:

    • MDIs may be at least equally effective if not more effective than nebulizers.
    • MDIs are also cheaper and provide an opportunity to educate patients regarding their use.
    • It is important to re-assess your patients following initial treatment.
    • There may be variability in the way you approach each patient to meet their individual needs. 
    • Always refer to your local guidelines.

    Being a woman in podcasting can feel like a really isolating experience sometimes. But being part of the minority in a super open and welcoming community makes it so much easier. Ali from Dungeon Fables joins us to talk about being a woman in Warcraft, her time podcasting, being an awesome part of the Warcraft community!

    Ali shares why she first started podcasting about Warcraft and what led her to take a very lore focussed approach. Dungeon Fables is a completely unique show because Ali looks at one dungeon per show, does days of research, and talks about the lore and background about it. 

    In fact, Ali says that between Dungeon Fables and the other two shows she co-hosts, All Things Azeroth and BlizzMates, she is working on Warcraft podcasting every single day. WHEW!

    Ali's podcast may be relatively new to the scene, but she's been welcomed to the Warcraft community with open arms. Because if there's one thing that community does, it's be a friendly, happy, and open to new people bunch. FOR THE MOST PART.

    Ali shares about some of the times she's been objectified while playing World of Warcraft and what the experiences have meant to her. Thankfully, they're few and far between, and being thirty-something years old now, these brushes with incels roll right off her shoulders. AMEN TO THAT!

    And excitingly, Ali gives us some pretty accurate stats about the ratio of women to men in Warcraft podcasting - and the difference isn't as shocking as you might think! AMAZING!

    Rising up the ladder, hitting Legend, topping the Badass Women of Hearthstone league, and competing weekly in the Hearthstone GrandMasters is a journey, but not one without an emotional rollercoaster. We're joined by Pathra Cadness who's sharing her experience as a professional eSports athlete, currently competing at the GrandMasters!

    Pathra is a Thai/Kiwi Hearthstone player who started streaming on Twitch back in 2015. She quickly climbed the ladder and started making a name for herself on the Hearthstone scene. When a friend convinced her to start competing with the Badass Women of Hearthstone, she soon started topping their leagues. Of course, Blizzard noticed, and now she's a regular feature in the Hearthstone GrandMasters, a weekly tournament run a couple times a year.

    Now that Pathra has moved to Atlanta, Georgia, this competition has ramped up on another level. Because she still competes in the Asia-Pacific tournament, Pathra is waking up and competing sometimes at 4 in the morning - crazy or what!! Imagine that pressure, especially when you're not firing on all cylinders! 

    In this episode, we're talking about Pathra's experience at the GrandMasters. It's not easy sailing being the only woman competing at this level. She breaks down what it's like reading the comments and Twitch chat that isn't always (or even usually) very complimentary of her.

    There's no doubt about it, playing Hearthstone at a professional level is nerve wrecking! It's 50% talent, 50% getting your head in the right space - and when you have harassment and trolls berating you in chat, it can definitely be challenge.

    Join us as Pathra gets intimate about her experiences, how we think Twitch and the GrandMasters can be improved and moderated better, and what's next for this Hearthstone Pro.

    Running two successful Blizzard positive podcasts, building a super supportive community, and working in gaming, all while dealing with anxiety and depression can definitely be hard! But our guest today, the incredible Thyst of Lagging Balls, makes it all look easy and completely natural.

    Thyst says that the best thing about everything she's doing is the community her and Laggin Balls co-host, Thorn, have built. They have an extremely active Discord channel where members of their community can share everything from memes and nudes to Blizzard game chatter to asking for help and support. It was really important to Thyst to have the kind of community that builds each other up.

    Thyst shares why she started podcasting and how she's built it from the ground up while dealing with some mental health issues and depression. In fact, World of Warcraft and podcasting is what helped bring her out of her depression -- who says gaming is bad for you! 

    You can say that Thyst has definitely found her spark, which is a GOOD THING because holy crap this woman is BUSY! She recounts a "day in the life" with a typical Tuesday, where she's out of her house for 12 hours a day talking about video games before coming home, shovelling food into her face, and recording Convert to Raid.

    Thyst gives some extremely valuable advice to content creators for getting industry jobs. If you've been wanting to take the leap from content creator to professional, you should definitely check this episode out! Super, super useful!

    Did you know that Thyst has started working for Blizzard? All her hard work and real life rep grinding has paid off: she was chosen to sideline report at the Mythic Dungeon International in Australia. A literally dream come true!

    Thyst talks about her experience as a female Warcraft podcaster, which for the most part has been pretty damn positive! Though she does get the occasional hateful comment or trolling, her community is so full of positive faces, both male and female, that she doesn't let them get her down.

    Her final thoughts? Find your passion, make it work, and get paid doing it!
