    A Pharmacist’s Take: Pharmacy Deserts and Patient Care – Part 2

    Today On Medical Grounds we will be speaking with Dr. Heather Whitley for part two of our discussion. Dr. Whitley is a Clinical Professor in the Department of Pharmacy Practice at the Auburn University Harrison College of Pharmacy. She is a Board Certified Pharmacotherapy Specialist and a Certified Diabetes Educator. Earlier this year, Dr. Whitley spoke with us about screening for diabetes in high-risk individuals. Dr. Whitley is well published—predominantly in diabetes related research.

    In part 1 we explored options for patients with diabetes who can’t obtain their GLP-1 receptor agonist drugs due to a nationwide storage. In part 2 we expand this conversation by bringing in the concept of pharmacy deserts and the pharmacist’s role in patient care.

    Antibiotics are lifesavers. These 20th-century drugs can save lives within hours, often given as just a handful of pills or an injection. 

    But the germs they fight often fight back and evolve quickly. They develop what’s known as antimicrobial resistance, and then the drugs people use to treat infections become worthless. 

    This happens naturally, but people and the way we behave often help it develop faster. When people use drugs when they don’t need them, or take the wrong drug, that allows bacteria, viruses, or fungi to evolve new tricks to evade the effects of medicines. 

    These drug-resistant superbugs kill millions of people every year, and the problem is getting worse. 

    The One Health Trust is working to educate medical professionals about better ways to conserve and care for these precious antibiotics. Policies and practices crafted to ensure that antibiotics, antivirals, and antifungal drugs continue to work are known as antimicrobial stewardship. 

    Dr. Azra Hasan, the Head of Academic Programs at OHT, is leading the One Health Trust’s efforts to help physicians, pharmacists, and others learn ways to better preserve antimicrobial drugs. 

    In this episode of One World, One Health, Azra explains first what the problem is, and what to do about it.

    65B. Cornell Conference: New Revelations in Transition Cow Nutrition with Dr. Santos

    Joining together for the second episode of the New Revelations in Transition Cow Nutrition from the 2022 Cornell Nutrition Conference four-part mini-series to discuss animal nutrition requirements are remarks from Dr. Jose Santos, University of Florida and topical insights from Dr. Clay Zimmerman from Balchem. 

    Bringing forward the most recent research on choline supplementation, Dr. Santos began the second in the series focusing on nutritional mechanisms and their essential benefits in animal growth and performance. 3:45

    Choline was first introduced in the 1700s by chemists and pharmacists. Still, it wasn’t until about 40 years ago that Derek Lindsay from England discovered that most phospholipids in ruminants are synthesized de novo. 

    Dr. Santos mentioned that studies show more than 90% of choline in feeds doesn’t show up past the rumen, adding to the lag in understanding when the essential nutrient shows up in the small intestine. 5:32

    As a required nutrient, Dr. Santos shared that choline is required for the structural integrity of cell membranes, neural tissue and the components of phospholipids and sphingolipids. 7:01

    As cows approach calving and during the first two weeks of lactation, Dr. Santos suggests that it is the optimum time to provide choline as a building block for phosphatidylcholine. 17:35

    Sharing a heat map study on the effects of choline in hepatic tissue, Dr. Santos said it ultimately shows that supplementing choline reduces glycerol and increases the synthesis of phosphatidylcholine. He added that as studies focus on hepatic triglyceride, acid basis or dry matter basis, the benefits of choline at different dosage levels benefit the same.  22:30

    But do low-body condition cows react to choline supplementation the same as high-body condition cows during the transition period? 

    Dr. Santos shifted directions, adding that he’s found low body conditions cows responded to choline with more milk and energy. He added that studies show supplying choline to nutrient-deficient animals enhances their ability to transport and absorb nutrients from the gastrointestinal tract. 41:55

    Dr. Santos mentioned not only does choline facilitate phospholipid synthesis and plays a large role in the transportation of fatty acids, he believes it’s an unquestionable supplementation that fits the requirements as a required nutrient.  46:35

    Wrapping up the conversation, Dr. Zimmerman highlighted key points from Dr. Santos and summarized consistent responses the industry continues to see in the meta-analysis of added choline in a range of production levels. 49:15

    We hear this message all the time: compounding pharmacies and the bioidentical hormones they produce are dangerous and unregulated. But, are they? To find out Ann Marie turns to two people who are inside the industry, which accounts for about 1 to 2 per cent of drugs sold, and gets a very different story. They are Scott Brunner, CEO of the Alliance for Pharmacy Compounding, which represents compounding pharmacists and technicians as well as prescribers, educators, researchers and suppliers in the United States, and Jennifer Burch, a doctor of pharmacy and owner of the independent pharmacy her father started in Durham, North Carolina. Although this episode focuses on the compounding pharmacies industry in the US, there are takeaways that can be applied all over the world.
