    Dr. Lee-Valkov is also a life coach, passionate about boundary setting and contract negotiation work. She helps people gain clarity on what is important to them, set and enforce boundaries, and negotiate for what they desire so that they can live a life in alignment with their values. She also works with graduating residents and fellows and other physicians considering career or practice changes.

    In her free time, Dr. Lee-Valkov manages several real estate holdings, including a short term rental. She also enjoys kayaking, SUPing, hiking, and making latte art. She is also an entrepreneur and is currently bringing her Trini style hot sauce, "Rayz Peppa sauce," to the market.

    Creating a Model for Healthcare in Rural America - Dr. Mike Waldrum, Dean of the Brody School of Medicine at East Carolina University and CEO of ECU Health

    Twenty years ago, health outcomes in Eastern North Carolina lagged behind state averages but those deficits have largely been erased, and Dr. Mike Waldrum, Dean of the Brody School of Medicine at East Carolina University, thinks he knows why. “We’ve done it primarily with a community-based focus and taking students only from North Carolina that we know have a propensity to want to practice medicine in the environments that we're here to serve. That's kind of our sauce,” he tells host Shiv Gaglani. Building on that approach, the university started a rural residency program in recent years, and out of four graduates in its first cohort, two have agreed to stay in the communities in which they trained. And while that kind of incremental progress on the ground level is important, Dr. Waldrum knows change is needed at the system level as well. Some things ECU Health can do on its own, such as implementing a unified electronic medical record across all of its care sites that allows it to model where health needs are and intervene early. But what he sees as the necessary restructuring of how the healthcare system is organized and financed will take a group effort. “We need Medicare, Medicaid, the insurance industry and others as partners in how we transform the system.” Tune in for a thoughtful look at the challenges of improving healthcare in rural communities, and the special role academic “safety-net” health systems play in that effort.

    Newcastle's veteran sports reporter Sonja Karp (we call her the "Ruler of the Sports Galaxy") goes off the page with Bob and Alexis to bring Dogie nation up to speed on the state of Newcastle High School athletics. The award-winning Karpe Dogie columnist talks about the year that was and the year to come at NHS, including some memorable campaigns for some Lady Dogie athletes last year and the high hopes parents, fans and coaches have for the incoming group of athletes. While we were at it, we talked about how hard it is for Middle School success to translate into high school championships in Newcastle, and the newspaper's job to look for the positives in our programs and young athletes to encourage support from the community. Hopefully that won't be a challenge in the coming year, as Sonja throws out some promising reviews (spoiler alert!) of the Dogie/Bobcat wrestling team, and hints that good things are coming for Dogie hoops and swimming programs as well. Alexis also brings us up to speed on a really big week at the hospital — and the budget to match. We also talk about modern cows and new-fangled ranching...not that we really know anything about that stuff!

    “We Want to Be the Most Inclusive University in the Country” - Dr. Janelle Sokolowich, Academic Vice President and Dean of the Leavitt School of Health at Western Governors University

    After battling chronic illness as a child, Dr. Janelle Sokolowich swore she’d never step foot in a hospital again and started pursuing a different path in college. But life had a way of bringing her back to the world of medicine. “I started thinking back to all the nurses that made such an impact on me as a child and helped me to grow up to be a functioning adult,” she explains to host Michel Carrese. Now as academic vice president and dean of the Leavitt School of Health at Western Governors University, Dr. Sokolowich is in a position to help many others like herself pay it forward by becoming healthcare providers. The school serves 20,000 to 30,000 students per month and seventeen percent of all BSN holders in the country are graduates, but Sokolowich is keenly aware this can be an unattainable dream when cost and other factors come into play. That’s why Leavitt strives to keep tuition low and her performance as Dean is evaluated based on how much debt students have when they graduate, and if they are earning a livable wage two years post-graduation. “We want to be the most inclusive university in the country and we see ourselves as personally responsible for advancing health equity through education.” Check out this thoughtful conversation about competency-based education, strategies for meeting health needs in rural America and the importance of mentoring. “I want to build that next set of nurse leaders and I take it personally, because I have been gifted and blessed with many that have done it for me.”

    Adam: Is Ron DeSantis wishing he’d never picked a fight with a mouse? 

    1. Josh Hawley (name drop) Is A Man, Bro. 
      1. In his new book, Josh Hawley, the Sr. Sen. for Missouri, not from, is ready to transform America by calling on men and boys to lead.
      2. HOW? 
      3. Great question - it seems, mostly, that Hawley is fixated on sex. Men should stop masturbating and porn should basically be illegal. 
      4. Oh, he must be super concerned for the women being exploited then..
      5. NOPE… just with Men being weakened by lust. Hawley is up for reelection in 2024 against Lucas Kunce. You may recall Kunce from his 2022 campaign for the Democratic nomination and a particular video in which Kunce climbs a very tall poll in his shoes and using his hands. Based on Hawley’s book and his assertion of being the arbiter of manliness, we have to assume the 2024 Missouri senate race will be a series of feats of strength culminating in a caber toss right out of the scottish highland games in which the candidate who throws a tree the farthest will win the seat. No word yet from the Hawley campaign on if Josh’s incredibly soft hands can handle the splinters, we will monitor this one closely. 
    2. In more useful news… The federal Rural Health Clinic program started as a way to provide safety-net care to underserved populations.

      2. Waylon Williams (great name) was born in Eastern Kentucky and at just 5 weeks old is already doing his name proud. Waylon is the first child born at a new facility in rural eastern Kentucky called “Beacons of Hope” a temporary housing facility for women facing substance abuse problems., part of the primary care centers of eastern Kentucky.
      3. Beacons of Hope is an extension of PCCEK’s Pregnancy & Beyond, an addiction-treatment program that offers obstetrical services, medication for substance use disorder, prenatal education, pediatrics, and counseling – services that in so many rural communities nationwide are in critically short supply or entirely absent. 
      4. The town of Hazard, where the largest of PCCEK’s four clinics is located, is in Perry County. Perry County ranks 117th among Kentucky’s 120 counties in health outcomes. Life expectancy is 67, as compared with 78.5 for the country. 
      5. Great to see some blue solutions helping out in the blue grass state
    3. Money and Power, as usual, go hand in hand

      2. The U.S. Department of Agriculture will begin to administer two loan and grant programs worth nearly $11 billion to boost clean energy systems in rural areas
      3. Rural areas can have more difficulty than more urban ones in attracting private sector investment, White House National Climate Advisor Ali Zaidi said. The programs are intended to allow those rural areas to take advantage of an industry-wide trend to invest in clean energy production.
      4. “There’s a favorable wind blowing here,” he said. “This allows rural communities to put up a sail.”
      5. The programs are meant to put rural electric cooperatives on equal footing with larger privately owned companies that have already put major funding into clean energy deployment, Vilsack said.
      6. The programs represent the largest single funding effort for rural electrification since President Franklin Roosevelt signed the Rural Electrification Act in 1936, a USDA press release said.
      7. The money is meant not only to address the climate impacts of fossil fuel energy and reduce home energy costs, but to act as an economic engine for rural areas, Zaidi said.
      8. So get ready folks, because that leftist liberal woke power is gonna be indoctrinating your meter before you know it. 
    4. Conservatives concerned about right wing extremists impact on schools

    6. Chief among those concerned? Conservatives (whaaat?)
    7. That’s right, it seems actual conservative folks have finally figured out that the MAGA members of their party might not be the best people to put in charge. 
    8. Lose Your School, You Lose Your Town - is the headline
      1. Contrary to Lobbyist Social Media Postings, Rural Schools Are At Major Risk, and educators in rural states are not sitting this out.

      3. Rural schools are also hubs for community engagement through concerts, theatrical productions, and sports. Often, they are a town’s largest employer.  
      4. “At our school, we offer a lot, because our community expects a lot,” says Steve Peterson, a teacher in Decorah, a town in northeastern Iowa. “They want good programs—academic, but also extra-curricular opportunities.”

    Tele-Palliative Care

    Twenty percent of Americans live in a rural setting and full access to some medical care is limited: often requiring long drives or temporary relocations. That includes specialty palliative care -- the interdisciplinary services of physicians, nurses, social workers, and chaplains aimed at helping patients control symptoms of a serious illness.

    On today's podcast, we explore a demonstration project underway in rural Stevens County, Washington undertaken by the Palliative Practice Group at the Providence Institute for Human Caring.

    The in-person nursing and chaplaincy providers in two rural hospitals are being joined by physicians and social workers through a Tele-PC video connection or by phone, allowing patients to avoid 70-100 mile drives to seek consultations.
