The final step in an open rhinoplasty procedure is an important one. The closing stitch ties all the surgical changes together, and in an open rhinoplasty is important to be done carefully so as to leave as least visible of a scar as possible. Taping and cast application then sets the nose up to begin healing in the desired shape.

Watch rhinoplasty specialist Dr. Buonassisi close the rhinoplasty surgery for his patient Ola with dissolvable stitches and a cast as he explains his process.

Sharpening a bulbous nose tip and shortening a nose that extends out from the face a little too far are two very reasonable outcomes that can be achieved with rhinoplasty surgery. Known as tip refinement and deprojection, these techniques are difficult to execute correctly without access to the nose cartilages around the tip.

Board Certified facial plastic surgeon and rhinoplasty specialist Dr. Buonassisi is an expert in the open rhinoplasty approach, which is the ideal approach for safely making changes to the nose as in tip refinement or deprojection.

Hump reduction is one of the most commonly desired outcomes in a rhinoplasty surgery. With thousands of surgeries performed and countless hump reduction techniques executed for patients, Board Certified rhinoplasty specialist Dr. Buonassisi is expert in this procedure.

Whether a slight crook off to the right or a visibly deviated septum, a septoplasty technique is what will be employed to help straighten the nose in a rhinoplasty surgery.

Done via an open rhinoplasty approach, facial plastic surgeon Dr. Buonassisi executes a septoplasty for patient Ola, who's nose slightly leans to the right. Listen in as he talks us through the surgical process of straightening the septum in a live operation.

Often stigmatized for its invasiveness; negative effect on recovery; and visible scarring, open rhinoplasty is a surgery procedure technique that facial plastic surgeon Dr. Buonassisi executes with expertise to allow for the most extensive possibilities in creating alterations to the nose to get the best possible results for the individual patient. Among the benefits of open rhinoplasty are greater precision and control when directly viewing the nasal structure, and better access for any manipulation of the nose tip.

The primary difference between the open and closed approach is found in the incisions, where open rhinoplasty incisions are made through the columella (hence the fear of visible scarring), and closed rhinoplasty incisions are made on the inside of the nose in the nostrils.

On surgery day, your plastic surgeon will go over the surgery plan with you one last time before going into the operation. Ola chose Board Certified facial plastic surgeon Dr. Buonassisi for her rhinoplasty procedure, who lays out the plan for her again with the techniques they have agreed on to get her the best results.

Will I really be awake during rhinoplasty surgery?

Anethesia options for rhinoplasty can vary in level, and you can certainly (and preferably) be awake during the procedure safely and comfortably when administered IV twilight anesthesia by a skilled anesthetist, like Dr. Buonassisi will often opt for.

Today, Dr. Buonassisi answers patient Ola's last few concerns about conscious sedation for rhinoplasty before her surgery.

Josh met Crystal at work about 5 years ago and immediately they became best friends.  Crystal doesn’t hide anything from Josh, so when she decided to have her upper eyelid surgery by Dr. Mendelsohn, he was instantly skeptical.  Josh didn’t want Crystal to look any different and was worried about the actual procedure.

Rhinoplasty can be performed "open" or "closed", with the latter often also known as "scarless" rhinoplasty. Speaking with Director of 8 West Clinic, Angie, Dr. Buonassisi details the difference between the two types of rhinoplasty surgery. In regards to the question of whether one or the other is the better approach, Dr. Buonassisi talks about common misconceptions around scarring, invasiveness, and overall effectiveness to come to a conclusion.

Oh, that infamous pointed "Hollywood tip" that we all find on magazine covers or advertisements and the like. What happens when you ask your plastic surgeon for a celebrity nose job? Dr. Buonassisi talks about how while it's a nice idea to come in to your consultation with some celebrity inspiration, it's not the best to expect major transformation to be possible with rhinoplasty surgery. Many factors go into the scope of what is achievable with rhinoplasty for each and every patient, and Dr. Buonassisi goes into this.

There are certainly a number of common requests that many patients desire out of a rhinoplasty. While they are popular to ask for, they aren't always a good idea depending on the patient and their nasal features. Dr. Buonassisi describes some examples of the requests he often receives from patients and why they might not exactly be reasonable to ask for.

Caring for your nose after having your cast removed post-rhinoplasty comes with many do's and don'ts. We bring you into the clinic at a real cast removal appointment one week after surgery, where Dr. Buonassisi discusses in-depth with his patient Ashley the things to keep in mind for caring for your nose after the cast comes off, including points related to cleaning, makeup, exercise, diet, and more for the next 6-8 weeks.
