    I’m talking about something near and dear to my heart in this episode: the journey through perimenopause. If you’ve been listening regularly you’ll know I’m on this journey right now and have been struggling with it. Why do we not talk about perimenopause regularly? Why is it so hard to get the information we need? That’s going to change in this episode because I’m talking with Amita Sharma, co-founder of Nourish Doc, a global holistic wellness platform for peri-to-post-menopausal women.     Amita speculates that information and discussion around perimenopause and the menopausal journey as a whole are largely taboo because women are still thought of generally in terms of fertility. With menopause, our fertility and reproductive era ends and society’s overarching thought is that we’ve had our day. We haven’t had our day, we’re still very much here, and very much in need of open conversations about the menopausal journey. So Amita and I define perimenopause and menopause, common symptoms, practical issues like hot flashes and wearing a diaper due to sudden blood flow, sleep issues, and all the ways Ayurvedic health practices can help us through this stage. From herbs to self-massage to oil-pouring sleep treatments, Amita shares knowledge on everything we women didn’t know we needed to know about perimenopause.     Day raver, cold swimmer, rabble-rouser, educator, author, podcaster, academic and publisher of scientific papers: Joyce Harper is a professor of reproductive science at the Institute for Women's Health, University College London. 

    She’s also author of the 2021 book Your Fertile Years and hosts the podcast Why didn’t anyone tell me this?

    Harper is uncompromised and unflappable. She is all about the evidence. She’s not opposed to HRT but isn’t a breathless enthusiast either. It has to be about the evidence, she argues, otherwise it’s just stories and anecdotes that don’t add up to anything. 

    NB: We are both members of a balanced-approach group called MenoClarity, and we recorded this podcast before Joyce was quoted in a controversial Daily Mail article alleging excessive and unsafe HRT prescribing practices at a chain of private clinics in the UK. 

    When I followed up with her about it, she said: "We need to give women accurate information about the menopause and treatments. In the UK we've created huge negativity – some suggesting menopause is a disorder. It's not fair to women and I really worry for the next generations who are now nearing menopause. Menopause can be difficult for some. But for most it's a transition and life post menopause can be the most exciting and liberating stage of our lives."

    A woman's midlife transition from peri-menopause through post-menopause brings a period of vulnerability for physiologic and symptomatic changes affecting both physical and mental health. Estrogen has multiple effects on neurotransmitter systems and brain regions including the hypothalamus, neocortex, hippocampus, and brainstem. During times of estrogen fluctuations and decline, dysregulation of neuroendocrine systems can lead to physiologic effects such as hot flashes, reduced bone mass, and depression. In a recent online survey from The Menopause Epidemiology Study, the reported prevalence of VMS, including hot flashes, night sweats, and sleep disturbances, was 79% in peri- and 65% in post-menopausal women. Osteoporosis, characterized by a progressive reduction in bone mass is linked to female gender, increased age, and estrogen deficiency. The transition to menopause has also been demonstrated to be a period of increased risk for depressive symptoms even in women with no prior history of depression. Vasomotor symptoms, osteoporosis, depression—each can impair a woman's functioning and quality of life. As women are expected to live well into their 80s, it is essential that healthcare providers recognize and manage these changes.
