Dr. Esmaili places a great emphasis on the relationship between oral health and overall well-being. She has undertaken multiple advanced education courses that delve into the connection between various aspects of the body, including gait, posture, the alignment of the jaw joint, and tension in facial muscles and how they relate to teeth structure and the airway. Dr. Esmaili is the Director of Clinical Education at Rinovive, Lead Clinical Director at Vivos Institute, and Strategic Advisor for the Council of Innovative Dentistry at 3M. Previously, she worked as a dentist for various companies and received a Doctor of Dental Surgery from the University of Colorado Denver School of Dental Medicine. Key Takeaways: What are TMJ and TMD? TMJ stands for temporomandibular joint, which is the joint that connects the jawbone to the skull. TMD is a term used to describe a range of conditions that affect the TMJ and the surrounding muscles and tissues. Some of its symptoms include pain or tenderness in the jaw, difficulty chewing or opening the mouth wide, as well as headaches or earaches. Teeth and total body health Teeth are not born with us but come into the equation later, and they can affect our overall body health. The hinge of our jaw is directly aligned with the two first two vertebrae in our neck. All the muscles making the hinge of the jaw happen are directly affected by the position of the lower jaw. So if the lower jaw is far back or is hiding on there, you have a neck issue. When the mandible is tucked underneath an underdeveloped maxilla, it can push against the airway, leading to breathing difficulties. This causes the head to go into a forward posture, resulting in neck issues and affecting the curvature of the spine from top to bottom. Environmental factors and facial development Environmental factors such as chewing functions, swallowing correctly with the tongue at the roof of the mouth, and breathing through the nose are necessary for facial development between ages four to seven. Lack of breastfeeding and tongue tie can lead to various issues in children, such as a stuck back jaw, pushing the tongue and palate against the airway, making it difficult for them to breathe. This leads to further problems such as jaw issues, TMJ headaches, and back problems. Understanding sleep breathing disorder The growing epidemic of mouth breathing in children due to underdevelopment of the sinuses and nasal passages can lead to symptoms such as noisy breathing, mouth breathing, snoring, sleep apnea, ADHD, fatigue, and brain fog. Some causes might include misalignment effects on the body, including masking pain with neurotransmitters, metabolic and neurotransmitter issues, an overactive immune system, or narrower airway difficulty reaching the deep sleep stage necessary for hormone regulation.

    Episode 28: Dr. James Carlson, DDS with Ron Hruska

    In this episode of the Postural Restoration Podcast,  I sat down with Dr. James Carlson who will be presenting at our 13th Annual Interdisciplinary Integration symposium on April 21-22nd. Dr. Carlson began studying gnathological principles while still a student at the University of Minnesota dental school nearly 60 years ago. While there, he had the opportunity to study under one of the top gnathologists at the time, Dr. Charles Stewart, and learn his techniques using the Stewart Articulator. From then on, he knew he wanted to pursue a dental career studying these principles. Following his dental schooling, Dr. Carlson spent 3 years in the Navy, eventually settling into his first private practice in Alaska. In his early years of  clinical practice, he began studying and applying the concepts of Centric Relation (CR) of the temporomandibular joints, and Centric Occlusion (CO) of the bite. Around this time, he was also introduced to the concept of "Dental Kinesiology", published by George Eversaul. These studies and others, allowed Dr. Carlson to see the influence that the neuromuscular system has on jaw position. Throughout these early years his "patients became his professors" and he has continued to learn from them and numerous other integrative minds throughout his career.

    Dr. Carlson began introducing these concepts within his appliance fabrication  and began to see an even greater influence of the neuromuscular system on dentition. His appliances at the time were meant to calibrate the maxilla to the mandible through the use of vertical height, anterior/posterior guidance and other qualities. This led him to the work of Dr. Barney Jankelson, who is widely known as the founder of Physiological Dentistry. From this point, his career was focused on Form vs. Function within the Stomatognathic system. This thinking led him to collaborate with not only numerous other dentists, but also included a 20 year partnership with an integrative chiropractor who also introduced him to the influence of Podiatry and the foots influence on posture. Through these collaborations, he was introduced to the Acculiner System which he spent many years selling and training others on its use. During this period he learned the importance of a level bite plane, and has promoted the concept ever since.

    Throughout his career, Dr. Carlson has authored numerous papers and publications. Starting off with "Beyond Dental Care", which he summarizes as "everything I wasn't taught in Dental School". We then discuss his original publication which was titled "Physiologic Occlusion" but was retitled "Orthocranial Occlusion & Dentition Design" following the release of a new edition. And "Analyzing the Stomatognathic System", in which the concepts of "Form vs. Function" of the entire system is addressed more globally. These publications played a large role in Ron's own study of the stomatognathic system and Dr. Carlson's experience and passion for these concepts were a primary factor in the decision to host this years symposium,  "The Stomatognathic System - An Interdisciplinary Approach in the Management of Spatial Navigation and Structural Strength". We end our discussion joined by Ron to discuss how they plan to navigate these concepts through their presentations next month. We are very excited for the opportunity to introduce our PRI community to Dr. Carlson and these topics as Ron Hruska, Dr. James Carlson and Jason Masek collaborate throughout this years symposium.

    In Episode #5, Daniel Burstein, Senior Director of Content & Marketing, MarketingSherpa, interviewed  Jonathan Fashbaugh, President, Pro Impressions Marketing (https://www.proimpressionsgroup.com/).

    Some lessons from Fashbaugh that emerged in the discussion:

    • Popular content isn't always the best content for the client. His team blogged on a dentist’s website about the vampire fang effect that was used in the TV show “True Blood.” The post explained how people could get porcelain veneers that looked like fangs…only problems was, the dentist did not offer that service. 
    • Well-written content put together by someone who understands the business can go a very long way toward the client's success. Fashbaugh went to seminars with a dentist, shared a hotel with him, and was his hands-on patient for the courses they attended together. He used this course as experience to help inform the website he was building for the dentist. 
    • Attention to detail in a niche pays dividends. Dentists have taught Fashbaugh to be very picky. There was a stock photo floating around that many design companies were using. But if you looked closely, the model was missing one of her teeth toward the back of her mouth. 

    Fashbaugh also shared lessons he learned from the people he collaborated with in his career:

    • Robin Schmachtenberger, Owner, idolum – Don't go around your supervisor, and you don't throw your weight around as the new person. 
    •  Bill Fukui, Senior Director of Business Development, MedShark Digital – Always get the client or prospect on the phone before you give up the goods. 
    •  Jeff Haddad, Owner and Dentist, Rochester Advanced Dentistry – Communicating with consumers in their language.

    How to treat TMJ (Temporomandibular Joint) Dysfunction and BRUXISM (teeth grinding) with YOGA

    What is TMJD?
    Temporomandibular dysfunction is a problem in this joint and may end up causing jaw pain, headaches, neck pain, limited mouth opening and clicking when moving the jaw.

    What is bruxism?
    Bruxism is the habit of teeth grinding, usually during sleep. Bruxism is an abnormal clenching or grinding of the teeth, either while awake or during sleep. Bruxism may play a role in temporomandibular disorders (TMD), although the exact link is unclear. 

    Florencia does it through workshops, classes and individual sessions with the Be Soma method that combines body exercises based on yoga and muscle chains, the development of interoception and mindfulness. This is an integrated mind-body balancing technique. This method is based on our understanding and experience of working with the human body {physical and energetic}. The SOMA technique allows us to access relevant information from the mental, physical and emotional spheres, delving into the deepest layers of the subconscious mind, to help identify and clear the origin of imbalances and triggers which contribute to illness. Through a new specially designed technique, she creates an individual program to help each person to enhance their healing ability, helping them to access and achieve their full potential, ie returning their original blueprint. Scientific research suggests that most illnesses originate in the energy field.

    Although she works independently, she comes from the corporate world where she worked for 8 years in communication departments. Currently, she does do some short stories and media collaborations on health and wellness and would like to continue developing.

    Sleep Technologists are licensed to conduct and score sleep studies. He is the
    founder of the Bite, Breathe and Balance Podcast, dedicated to the
    multidisciplinary approach to treating craniofacial pain and sleep disorders in
    adults and children. Dr. Shirazi is the director of state of the art private
    practices, The TMJ and Sleep Therapy Centre of Conejo Valley and Los
    Angeles, that are limited to the treatment of TMD, craniofacial pain, Sleep
    breathing disorders, and craniomandibular orthopedics.

    His practice is part of
    the TMJ and Sleep Therapy Centre international family, one of over 65
    Centres throughout the world, and is located in the beautiful hills of Thousand
    Oaks and Brentwood CA. Personally, Dr. Shirazi enjoys hiking and camping
    in the state parks, traveling, and speaking. He is married to the love of his life
    Kimberly Shirazi, and have welcomed their first son, Maximus. They live in
    the Santa Monica Mountains, where they are very close to nature.

    REVIEW: PBM for Dentistry

    While focused on dental applications like TMJ pain, xerostomia, and oral mucositis, this systematic review makes some very good points for light practitioners of any discipline. The researchers provide a look at the need to view deep tissue targeted laser therapy in tissue volume, rather than surface area. Dr. Rountree goes over the additional recommendations for proper spot size, energy dosing, and laser handpieces to achieve optimal tissue healing, whether the target is superficial or deep.
