    Episode 262 | How To Develop A Stepped Care Model For Pain Management With Matthew J Bair, MD

    In this episode, we were discussing how to build a Stepped Care Model for the treatment and intervention of chronic pain. The Stepped Care Model for chronic pain originated in the VA health system and has been used in a number of other places. This model prioritizes the role of primary care providers in optimizing pharmacological management as well as the timely and equitable access to patient-centered evidence-based non-pharmacologic approaches.

    Joining us to discuss this model is Dr. Matthew Bair. His principal research focuses on chronic pain, psychological comorbidity, and developing strategies to improve pain management in the primary care setting. He has a strong background in conducting clinical trials, developing and testing interventions that combine pharmacologic and non-pharmacologic treatments, and his funded work often contrasts pharmacological and behavioral approaches for pain management.

    He has served on several national Veterans Affairs committees related to improving pain management as well as the Clinical Practice Guideline Committee at the American Pain Society. He serves as an Editorial Board Member for Pain Medicine and the Journal of General Internal Medicine. In retaliation for her disclosures, she was stripped of her role as a public affairs officer, one she held for 20 years. Over the next few years, Paula fought the system – and won. She earned her title back, was promoted to handle national public affairs for the Veterans Administration, and retired with 40 years of service, including her 10-year service in the Navy.

    58. Sprayed and Betrayed

    This is the story of a dangerous toxin released on our enemy and our own, all the while being lied to by the government. No, we’re not talking about Covid, we’re talking about Agent Orange.

    Much of South Vietnam is covered with very thick, dense jungle vegetation. This dense vegetation was used as cover by the forces opposing the United States and South Vietnamese armies. One means of counteracting this tactic was the use of herbicides to kill and defoliate vegetation.

    Agent Orange was a powerful herbicide used by the U.S. military during the Vietnam War to eliminate forest cover and crops for North Vietnamese and Viet Cong troops. These chemicals were applied by airplanes, helicopters, trucks, riverboats, and individual backpacks. Three million acres of South Vietnam were sprayed with 50,000 tons of Agent Orange and other herbicides, containing over 500 pounds of dioxin. Because herbicides were used in Vietnam to destroy crops and defoliate jungle cover, the military used higher concentrations of herbicide per unit area than most domestic applications, which are primarily used to prevent weed growth. Dioxin is a by-product of herbicide production and is the focus of veterans’ continuing health concerns today because of its deadly toxicity.
