    Wali S.: intrahepatic cholestasis

    Dr Alex Knisely today is speaking with Dr Sami Wali, of Riyadh, Saudi Arabia, senior attending gastroenterologist and chief of transplant hepatology at Prince Sultan Military Medical City. Dr Wali rose through the Saudi educational and medical-education systems, training at several Riyadh hospitals, with specialisation in paediatrics and in paediatric gastroenterology and hepatology that culminated in a year in Ontario at McMaster University and Hospital for Sick Children, Toronto – he is one of Prof Eve Roberts’ “old boys”. In Saudi Arabia many children with liver disease have metabolic disorders. Dr Wali has made large contributions to elucidating and to differentiating the features of these diseases, some of which were until recently unrecognised. Clinical, histopathologic, and genetic correlations in this field have been his specialty.

    At the 2023 ESPGHAN annual meeting in Vienna he presented his experience with the forms of intrahepatic cholestasis encountered in patients with tight junction protein 2 disease, a disorder unfamiliar to many but of great interest in hepatic pathophysiology. He shares that experience with us now.

    Advancements in Blood-Based Biomarker Tests for NASH — Interview with Dr. Tania Kamphaus and Tony Villiotti

    This episode features an interview with Dr. Tania Kamphaus and Tony Villiotti about new research on the utility and promise of blood-based biomarker tests for the diagnosis of liver diseases like non-alcoholic steatohepatitis (NASH).

    The research is particularly timely because the American Diabetes Association (ADA) recently called for liver disease screening for all adults with type 2 diabetes.

    The research, published in Nature Medicine, presents results from a study that evaluated blood-based biomarker tests for the diagnosis of NASH.

    The study’s co-author, Dr. Tania Kamphaus, Director of Translational Science, Metabolic Disorders, as well as Director of Patient Engagement, at the Foundation for the National Institutes of Health (FNIH), shared results from the study in this podcast episode. She discussed the potential of non-invasive tests to become the standard of care for liver disease diagnosis and the ongoing work her group is doing in this area.

    Tony Villiotti is the founder of the nonprofit, NASH Knowledge, and a patient whose NASH led to liver cancer and subsequent liver transplant. He spoke about the impact this research will have on the patient community. Tony believes that his liver cancer could have been prevented had a non-invasive test for NASH been available to him.

    A Double Donor’s Story of Loss and Redemption | EP 14

    According to United Network of Organ Sharing, only 133 living donors in the entire U.S. have donated both a kidney and a portion of their liver to two different recipients.  A few years back, this was very uncommon, but more recently, we have been seeing it in the news more and more.  Many transplant centers won’t even entertain the idea of letting someone donate twice, however other transplant centers are taking a second look, and essentially asking, well, why not?  If the donor is emotionally and physically fit to donate a second time, then let them donate.

    Today’s guest is double donor extraordinaire Lynn Bolduc who donated a kidney and a portion of her liver, both to strangers about 8 years apart.  Lynn tells an emotional story about a traumatic experience when she was 14, that ultimately led her down the path of organ donation later in life.  Her kidney donation story and her liver donation story are vastly different and illustrate that not all donation stories have the ending that we planned for.  Tune in to hear Lynn’s brave, bold, and vulnerable story about inspiring hope.

    Just in time for Liver Cancer Awareness Month, we interview Donna R. Cryer, JD Founder, President, and Chief Executive Officer of Global Liver Institute. GLI is the only patient-driven liver health nonprofit operating across the US, EU, and the UK. GLI convenes the NASH, Liver Cancer and Pediatric and Rare Liver Disease Councils, as well as the Liver Action Network, collectively more than 200 organizations. In the episode we chat with Mrs. Cryer about how she has channeled her personal experience as a patient with inflammatory bowel disease and a liver transplant recipient into professional advocacy across a career in law, policy, consulting, public relations, clinical trial recruitment, and nonprofit management. At GLI, Mrs. Cryer has raised more than $10 million for liver health initiatives. Among her many accomplishments with GLI, she developed a program featured by the White House on Solving Organ Shortage/Transplantation.     Women are drinking more than ever before, as much as men for the first time in history. Dr. Jessica Mellinger says today’s stressors make women an easy target by the alcohol industry. Dr. Mellinger is a liver doctor at the University of Michigan who sees more young women with liver disease, and she says a woman who drinks like a man gets sicker, faster. She hopes educating women about what moderate drinking really means, will turn this dangerous trend around.

    Sweating It Out: Exercise versus Toxic Exposures

    With nonalcoholic fatty liver disease affecting nearly a third of the US population, Luma Melo, University of Pittsburgh, describes how mouse studies have shown that low-impact exercise can help reverse liver damage. Dr. Melo also shares with co-hosts Anne Chappelle and David Faulkner how research funding works in her native Brazil and the role of Brazilian women in toxicology.

     

    About the Guest
    Luma Melo, PhD, University of Pittsburgh, started her scientific career in her native Brazil, earning a master’s degree in physics from the University of São Paulo, where she also served as an Associated Instructor. She then earned a master’s degree in the philosophy of science and medicine and a PhD in environmental health from Indiana University in the United States.

    Dr. Melo’s research focuses on liver disease (both alcoholic and nonalcoholic fatty liver disease) and exercise. She is exploring mode of action and genetic expression related to how exercise and environmental chemicals modulate the development of nonalcoholic fatty liver diseases, as well as liver fibrosis and the mechanisms and determinants of mortality of alcoholic liver disease through translational studies including human samples and experimental models of liver disease. She also has conducted research involving aerobic exercise and breast cancer and published a book on quantum physics for laypeople titled But After All … What Is Quantum Physics?

    Dr. Melo is currently a Postdoctoral Associate at the University of Pittsburgh Medical Center and an Adjunct Professor with Ball State University.
