    Service Without Sacrifice with Dimple Dhabalia

    Dimple Dhabalia began her career as a lawyer but after a stint at the Attorney General's office in Denver, she found her calling as an asylum officer. Working for almost two decades at the crossroads of the government and humanitarian sectors, Dimple found her job fulfilling, but it also led to vicarious trauma, compassion fatigue, moral injury, and burnout. 

    With a mission to put the word “human” back in “humanitarian,” she started “Roots in the Clouds” which - among other things - supports humanitarians around the world in challenging the narrative of service before self  and normalizes improving and protecting mental health in the workplace.

    174: Moral Injury

    While medication has focused on treating the physical injuries that can contribute to PTSD, it remains ill equipped to address moral injuries one may encounter in war. Beyond the stigma placed on military veterans, and a cruel incentive program directed at their mental health, the idea of PTSD, when reduced to merely physiological responses to trauma, only looks at half the problem.

    A moral injury is defined as "When one feels they have violated their conscience or moral compass when they take part in or fail to prevent an act that disobeys their own moral values or personal principles," by the DAV (Disabled American Vets) or as "When someone does something that goes against their beliefs this is often referred to as an act of commission and when they fail to do something in line with their beliefs that is often referred to as an act of omission. Individuals may also experience betrayal from leadership, others in positions of power or peers that can result in adverse outcomes (2). Moral injury is the distressing psychological, behavioral, social, and sometimes spiritual aftermath of exposure to such events (3). A moral injury can occur in response to acting or witnessing behaviors that go against an individual's values and moral beliefs." according to Sonya Norman and Shinra Maguen for the VA. 

    The effects of moral injuries can be mitigated by individuals with a strong understanding of their moral foundation, and a dedication to it that surpasses their circumstances.

    I returned to the airwaves after taking s me time off.  The original reason for starting the podcast was trying to grapple with the profound loss of a fellow physician, a dear friend whose suicide opened my eyes to the silent struggles we endure as healers. Her passing was a harsh wake-up call that sparked this podcast, aiming to shed light on the mental health challenges and burnout that too often go unnamed in our profession. In this poignant episode, I lay bare my own journey through the dark corridors of burnout, reflecting on the personal tribulations that accompany the life-saving work we do every day.

    As we navigate the year 2024, I invite you to connect with the raw, honest stories of being human from physicians who have teetered on the brink yet found solace and strength in their vulnerability. We're not just surviving; we're learning to thrive by embracing therapy, coaching, and the support networks that trailblazers like Dr. Londoño and Dr. Corrigan have put in place. Their initiatives, including Physician Coach Support and Physicians Anonymous, serve as lifelines, fostering a sanctuary for us to find camaraderie and the courage to seek help. This episode is more than a conversation; it's a call to reshape the culture of medicine, one heartfelt story at a time.

    2023 was a painful year, and the sentiment is that it's only beginning. 

    The question remains, what is the IT that is only beginning? What is the cause or explanation of the upheaval? Books have been written on the subject, and they pile up on bookshelves and in influencer's reading lists, but do they end up helping or perpetuating the problem.

    In this episode, I put forward the theory that we are experiencing the end of an age, the Secular Age as Charles Taylor puts it. In order to explain what that means we go into three different definitions for the word "secular" and what their end might mean.

    Shauna Hill, LICSW, Cofounder and CEO of mental health media and education company, StateChange, is a Vermont-based neuroscience educator and trauma psychotherapist with decades of experience in family, community, and behavioral health. Shauna joined the “Finding Joy” podcast to discuss strategies for addressing health worker stress and burnout. Shauna offers her decades-long earned insights into the healthcare worker crisis in “The Health Worker Crisis Is Real and We Must Act Now to Avoid Health Care Catastrophe”.     Shauna Hill, LICSW, Cofounder and CEO of mental health media and education company, StateChange, is a Vermont-based neuroscience educator and trauma psychotherapist with decades of experience in family, community, and behavioral health. Shauna joined the “Finding Joy” podcast to discuss strategies for addressing health worker stress and burnout. Shauna offers her decades-long earned insights into the healthcare worker crisis in “The Health Worker Crisis Is Real and We Must Act Now to Avoid Health Care Catastrophe”. She describes the kind of support with which healthcare leaders are in need and that of the health worker staff as well. From “finding a spark” to providing open discussions, Shauna believes, with immediate action, the health worker crisis can be overcome.

    “Finding Joy: The Health Care Professional’s Journey to Well-being" is a podcast resource developed by a team of interprofessional education researchers from Washington State University Health Sciences Spokane.
