    IRS: Pro-Pharma Anti-Health

    IRS: Pro-Pharma Anti-Health

    How the government, particularly the IRS, looks at exercise and diet versus weight loss drugs and other pharmaceuticals is discussed in this episode with Calley Means and Robert F. Kennedy Jr.

    Here is part of an official statement from the Internal Revenue Service website:

    WASHINGTON — Amid concerns about people being misled, the Internal Revenue Service recently reminded taxpayers and heath spending plan administrators that personal expenses for general health and wellness are not considered medical expenses under the tax law.

    This means personal expenses are not deductible or reimbursable under health flexible spending arrangements, health savings accounts, health reimbursement arrangements or medical savings accounts FSAs, HSAs, HRAs, and MSAs.

    This reminder is important because some companies are misrepresenting the circumstances under which food and wellness expenses can be paid or reimbursed under FSAs and other health spending plans.

    Some companies mistakenly claim that notes from doctors based merely on self-reported health information can convert non-medical food, wellness and exercise expenses into medical expenses, but this documentation actually doesn’t. Such a note would not establish that an otherwise personal expense satisfies the requirement that it be related to a targeted diagnosis-specific activity or treatment; these types of personal expenses do not qualify as medical expenses.

    About this interview:

    Dr Bob Gill NHS GP of 22 years, activist and producer of “The Great NHS Heist”. The movie was made at great personal expense in time and money, crowd funding £40000 and self paying an additional £20000. Bob is passionate about the NHS and universal publicly provided and available healthcare. Bob is also critical of the direction the NHS has been travelling over the last few decades, with mismanagement, waste, corruption and destruction of the doctor patient relationship. Bob highlights the increasing control of Big Pharma and the Medical Industrial Complex on the NHS and the failure (collusion) of health institutions to safeguard patients. Sarah Kliff grades Biden and Trump's health care plans

    Sarah Kliff grades Biden and Trump's health care plans
    There are few issues on which the stakes in this election are quite as stark as on health care. Democratic presidential nominee Joe Biden plans to pass (and Democrats largely support) a massive health care expansion that could result in 25 million additional individuals gaining health insurance. The Trump administration, as we speak, is pushing to get the Supreme Court to kill the Affordable Care Act, which would strip at least 20 million Americans of health care coverage.    There's no one I'd rather have on to discuss these issues than Sarah Kliff. Kliff is an investigative reporter for the New York Times focusing on health care policy, and my former colleague at the Washington Post and Vox where we co-hosted The Weeds alongside Matt Yglesias. She's one of the most clear, incisive health care policy analysts in media today and a longtime friend, which made this conversation a pleasure. We discuss:  The legacy of Obamacare 10 years later Why the fiercely fought over “individual mandate” isn’t all it’s cracked up to be  What Biden’s health care plan would actually do — and where it falls short  Whether a Biden administration would be able to pass massive health care reform — and why it might still have a chance even if the filibuster remains intact  The ongoing Supreme Court case to dismantle Obamacare  Whether Donald Trump has a secret health care plan to protect those with preexisting conditions (spoiler: he doesn’t)  The hollow state of Republican health care policy  The academic literature showing that health insurance is literally a matter of life and death  Which social investments would have the largest impact on people’s health (hint: it’s probably not expanding insurance)    And much more References: "If Trump wins, 20 million people could lose health insurance. Paul Krugman on climate, robots, single-payer, and so much more
    It’s cliché to call podcasts wide-ranging. But this conversation, with Nobel-prize winning economist and NY Times columnist Paul Krugman, really is. A sample of what we discuss: - How economists mucked up the climate debate - What a Democratic president should pass first - The politics and policy of Medicare-for-all - Krugman’s three-part test to determine whether a program needs to be paid for (don’t miss this!) - Why Pete Buttigieg is wrong on tuition-free college  - Why Andrew Yang is wrong on automation - What the Obama administration got wrong, and right, in the financial crisis - The means-testing vs. universal program debate is a false dichotomy  - What it would take to revitalize the economies of middle and rural America - The productivity puzzle - The antitrust problem - Geographic inequality - Whether elite or mass opinion is the key constraint on policy ambition - Path dependence in social welfare states - Whether private insurers should exist  And much more. #68 - Marty Makary, M.D.: The US healthcare system—why it’s broken, steps to fix it, and how to protect yourself

    #68 - Marty Makary, M.D.: The US healthcare system—why it’s broken, steps to fix it, and how to protect yourself

    In this episode, Marty Makary, Johns Hopkins surgeon and NYT bestselling author, discusses his ambitious attempt to fix the broken U.S. healthcare system through educating the public, changing the lexicon, encouraging radical transparency in pricing, and more. We go in detail into the main drivers of inflated health care costs, the money games being played making it hard to understand, and the unfortunate system structure that has resulted in one in five Americans finding themselves in medical debt collections which can ruin the lives of people and families seeking basic medical care. Marty also shares some very practical advice and tips if you find yourself a victim of predatory pricing and stuck with an outrageous medical bill. In the end, despite the current state of the system, Marty discusses the many exciting trends gaining traction in healthcare and why he is very optimistic and hopeful about the future.

    We discuss:

    • The science of delivering healthcare, how we need to do better as a system, and why no single person or entity fully to blame [10:15];
    • The stories that prompted Marty to write his first book (Unaccountable) [19:15];
    • The Surgery Checklist [26:15];
    • The problem is with the system (not any one person or entity) and the misaligned interests of all the parties involved [28:15];
    • Patients crave honesty and transparency, and the story of Peter’s back surgery gone wrong [33:00];
    • Today’s med students and young doctors have less tolerance for predatory pricing and healthcare industry BS [44:30];
    • Funny stories about John Cameron (legendary surgeon at Johns Hopkins) [48:00];
    • How doctors are trained to internalize traumatic experiences which can result in a misunderstood form of “burnout” [57:40];
    • The beat down of med students with traditional medical education and some exciting innovations to medical education [1:07:00];
    • Exciting trends in healthcare and an optimistic view of the future [1:11:30];
    • The Price We Pay (Marty’s new book), an attempt to illuminate the blackbox that is the US healthcare system [1:21:00];
    • Why it’s not always in the best interest of the insurance company to negotiate the best price [1:28:30];
    • Who is actually paying for medical costs, and Marty’s frustration with the healthcare lexicon [1:32:00];
    • Pros and cons of a single payer system [1:37:00];
    • How to fight outrageous medical bills and predatory pricing (and make a dent in the wasteful healthcare spending for the country) [1:49:30];
    • Disrupting the healthcare industry with private healthcare facilities with market demanded transparency [2:05:45];
    • The people hurt the worst by the current US healthcare system, the sad breast cancer statistic, and the importance of knowing that medical bills are negotiable [2:09:30];
    • The healthcare industry bubble [2:14:00];
    • Increased costs from unnecessary tests and procedures [2:16:30];
    • Malpractice concerns due to the litigious culture in America: What influence does it have on unnecessary testing, healthcare costs,  and overall quality of treatment  [2:22:00];
    • Drug pricing, price gouging, middle-men money games, kickbacks, and other drivers of healthcare costs [2:27:45];
    • How can we possibly fix the healthcare system? [2:34:30];
    • Helpful resources [2:46:15]; and
    • More.

    Matt Bruenig’s case for single-payer health care

    Matt Bruenig’s case for single-payer health care
    The Democratic primary has been unexpectedly dominated by a single question: Will you abolish private health insurance? Wrapped in that question are dozens more. Why, if private health insurance is such a mess, do polls show most Americans want to keep it? What lessons should we take from the failure of past efforts at health reform? What does it mean to say “if you like your health insurance plan, you can keep it?” Matt Bruenig, the founder of the People’s Policy Project, is firmly in support of true single-payer. No compromise, no chaser. He’s frustrated by those, like me, who try to work around the public’s resistance to disruptive change, who treat past failures and current polls as predictions about the future. And, in turn, I’m often frustrated by Matt’s tendency, mirrored by many on the left, to treat people with similar goals but different theories of reform as villains and shills. In this podcast, Matt and I hash it out. The questions here are deep ones. When are political constraints real, and when are they invented by the people asserting their existence? If you already believe the political system is broken and corrupt, how can you entrust it to take over American health care? Can you cleave policy from politics? What would the ideal health care system look like, and why? Book recommendations: A Theory of Justice  by John Rawls What Is Property?  by P. J. How Mitch McConnell convinced Michael Bennet to run for president

    How Mitch McConnell convinced Michael Bennet to run for president
    I’m not sure what I expected Sen. Michael Bennet’s answer to be when I asked him why he was running for president. I didn’t expect it to be “Mitch McConnell.” Since arriving in the Senate in 2009, Bennet has built a reputation as a senator’s senator. He’s smart and measured, thoughtful on policy, and good at working across the aisle. I’ve had colleagues of his tell me they wish he’d run for president, that he’s the kind of guy the country needs. But Bennet’s been radicalized. He believes America’s government is broken. So what happens when you radicalize a moderate? How far will an institutionalist go to save the institutions he loves? And at what point do you decide the problem is inside the institutions themselves? That’s the conversation, and at times argument, Bennet and I have in this podcast, and it’s an important one. His critique is angry and sweeping. But are his solutions as big as the problem he identifies? We also talk about his plan to end extreme childhood poverty, which I think is one of the most important proposals in the race, his view that rural America is the key to passing climate legislation, why he opposes Medicare-for-all, what to do about the filibuster, and much more.
