S2 E1.1 - How Recent COVID-19 News Affects NAFLD and NASH Patients

Manal Abdelmalek joins the Surfers to consider the three most important COVID-19 stories today.

The conversation focuses on the British variant, the new vaccines, and the physical and emotional toll the pandemic continues to take on front-line physicians and other healthcare workers. Stephen and Manal each have a sibling who works as an ER physician and they provide heart-wrenching stories on how tough the front lines are. Louise discusses how Britain is reacting to government policy designed to contain the new, more contagious COVID-19 variant. Roger discusses possible epidemiological trends and explains why the US may eventually follow the UK policy of delivering the first doses of the vaccine as quickly as possible. Thought-provoking in its own right, the discussion helps frame treatment and policy choices as well as the rest of this episode.

    The Pandemic Economy

    The Pandemic Economy

The economic collapse unfolding before our eyes is much bigger than it appears and the solution isn’t simply to “build back better.” COVID-19 didn’t create the challenges we face. It laid bare flaws that have long existed at the foundation of our system. That is particularly true for Black, Indigenous, and other People of Color. What kind of reconstruction effort would truly reflect the scale of the problem? What should we rebuild and what should we abandon? To answer these questions, Laura interviews Professor Robert Reich, economist Stephanie Kelton, community organizer Esteban Kelly and Service Employees International Union president Mary Kay Henry about their visions for a recovery that will enable us to build a new economy that is equitable, reparative and sustainable.  Music in the Middle:  “Stepping Back In Time” by Basel and the Supernaturals from their album “Smoke & Mirrors” courtesy of the artist.

2020 Year-End NASH Review 2: Patient Treatment and Perspectives

Guests Ian Rowe, Joern Schattenberg and Wayne Eskridge join the Surfers to discuss key lessons learned in 2020 about Patient Treatment and Perspectives.

Ian Rowe, Joern Schattenberg and Wayne Eskridge join the Surfers to explore some of the key lessons learned and issues uncovered in 2020. Ian Rowe discusses what population-level research in Leeds, UK, tells us about the benefits a streamlined liver testing model can yield not only in terms of better, faster diagnoses but also cost savings and improvement in patient satisfaction. Joern Schattenberg considers the implications when German mass media raises issues of Fatty Liver Disease in holiday season news stories. Louise Campbell questions what a patient-supported push to change the disease nomenclature from NAFLD to MAFLD implies for the future, while Stephen Harrison describes the emerging effort to get the most accurate and clearest nomenclature possible. Finally, Wayne Eskridge provides his perspective as patient and patient advocate on a year full of both happy surprises and disappointments and he finds himself surprised to be more optimistic at the end of 2020 than at the beginning.

“I Can Never Unsee, What I Have Seen.”

“I Can Never Unsee, What I Have Seen.”
My guest, Darlene Nelson, RN, founder of Expert Nurse Consultants, joined me on this show on April 13th along with Dr. Juan Nieto, ED physician. The description I gave then, STILL fits. “In the COVID-19 Pandemic in the USA, there are two fights going on. One against a virulent & dangerous new coronavirus & the other on behalf of the safety & protection of ‘essential’ workers across the country. ”The disease is doing an exceptional job at its mission: to infect & kill as many hosts as possible. The people, hospital administrations and political leaders are, to a large extent, failing miserably at their mission: To protect each & every healthcare worker and all essential employees.” We currently have passed 300,000 deaths (as of 12/15/20) and 17 million with the illness. We are nearing 3000 deaths of nurses (which is certainly an undercount.) Today huge numbers of Americans continue to ignore the admonitions to “Wear a Mask, Distance from Others and Wash your Hands.” How will this end?

What Your Fertility Nurse is Really Thinking

What Your Fertility Nurse is Really Thinking

Fertility nurses play an integral role in both our reproductive treatments and also our experiences at clinics. They’re the ones who do our ultrasounds, explain our medication protocols and often are the ones calling us with our beta results post IUI or IVF.

In this episode we’re talking to fertility nurse Dori Gelfman about why she got into healthcare, what makes fertility patients special and what she’s reallyyyy thinking when her patients break down and cry (or accidentally wear a jumpsuit to an ultrasound appointment).

The Surfers end the year on an optimistic note by talking with sometime-surfer Donna Cryer and GLI Director of Policy Andrew Scott about GLI's ambitious 2021 US NASH Action Plan, which should be released by the time you read this. The plan seeks to align all groups of key NASH stakeholders around common goals to provide appropriate diagnostics, therapy and support for NAFLD and NASH. The panel also considers time discussing acceptance and rate of uptake for the COVID-19 vaccines and the nature of leadership in a complex situation like the one NASH faces throughout the world. Dr. Peter Chin-Hong

    Dr. Peter Chin-Hong
We talk with Dr. Peter Chin-Hong from University of California, San Francisco about his expertise as an infectious disease clinician on the novel coronavirus and COVID-19. We'll talk about the current state of the pandemic in California, around the country, and in the world at-large. We also get into what's happening with the vaccines (including the FDA approved Pfizer-BioNTech vaccine that uses mRNA to rewire a genetic trigger to the viral proteins) as well as other therapeutics, pandemic fatigue, and how holiday travel might work as other get togethers. But we didn't just talk about the biology or healthcare. We also talked about how Peter — or as his students call him, PCH, which are his initials as well as California's famous Pacific Coast Highway — got interested in medicine while he was growing up in Trinidad and Tobago. We also talked about racism is a public health concern and how Twitter provided not only community for him this year but also a way to learn from others in the larger medical community. Stay tuned until the end of the episode for a funny bit waxing about technology problems while recording. Special Guest: Dr. Peter Chin-Hong.

COVID CHAOS: The 2nd Wave - Ep 39

COVID CHAOS: The 2nd Wave - Ep 39

Guest Ian Rowe and Dr. Kris Kowdley join the Surfers to discuss how the 2nd COVID wave and arrival of the vaccine are likely to affect patient treatment and clinical development.

In Episode 39, hepatologists Ian Rowe and Kris Kowdley, who divide their time more or less evenly between patient clinics and clinical trials, join the Surfers to discuss how the Second Wave of COVID-19 will affect patient treatment and clinical trials. This conversation turns conventional wisdom on its ear. The Surfers focused less on individuals' behavior and more on supply chain disruptions, growing shortfalls in the number of available healthcare workers and large-scale dislocations that will occur after the vaccine is in place. This episode challenges underlying assumptions about how the next few years might evolve and what will matter most.

Prolonged Disaster Nursing: Recognizing & Handling Trauma, Part 1

Prolonged Disaster Nursing: Recognizing & Handling Trauma, Part 1

"I became a nurse because I wanted to be bedside, I wanted to care, I wanted to touch and actually be next to a patient. Now, not being able to give that basic care that I think someone needs… it's discouraging, it’s very upsetting to not be able to do that.” – S. Curran, BSN RN

Disaster nursing should be quick and intense - and then over. What are the effects on caregivers when it continues for months? Nurse Shaina Curran and Dr. Jenny D’Olympia discuss the effects of COVID-19 on frontline nurses.

Topics discussed include: limitations and red tape around caring for patients, signs of stress and trauma nurses can look for, ways to decompress during and after a shift, feelings of anger, communicating and interacting with family, parallels to returning combat veterans, and self-care strategies.

    Expert: Shaina Curran, BSN RN
Shaina has been a traveling nurse for three years and has worked three COVID-19 contracts in Boston and across the country since March 2020. Before becoming an RN, Shaina was a hospice certified nursing assistant.

Expert: Dr. Jenny D’Olympia licensed mental health counselor with a PhD in Psychology.
Dr. D’Olympia has nine years in the US Air Force, five active and four reserve with several deployments. Through the Department of Veterans Affairs she worked with returning veterans and their families assisting with readjustment from war including feelings of anxiety, depression, and trauma. She is the daughter-in-law of a nurse!

    Everyday Courage for Extraordinary Times

    Everyday Courage for Extraordinary Times
Fear is a reaction, courage is a decision. These 8 powerful words are from my Guest, Joe Tye’s, new class, “Everyday Courage for Extraordinary Times.” Everything in our life--our successes, joys, mediocrity, losses, & failures can be traced back to decisions we have made along the way at critical moments. We are currently in a time of GREAT historic significance. The decisions EACH of us make (whether we SEE ourselves as people of power, or not important at all), WILL impact not only OUR future but that of our country & our world for perhaps centuries to come. It all comes down to COURAGE. Even if we do not feel particularly courageous now, we each have the possibility to LEARN Courage. My guest has developed this class, for people like you & me! This is an impromptu Episode with Joe Tye, Founder & Head Coach of Values Coach, a firm obsessively focused on helping hospitals build a positive culture of ownership with courses on values-based life & leadership skills.
