Surgical Sophistry: The 216th Evolutionary Lens with Bret Weinstein and Heather Heying

    en-usMarch 13, 2024

    Podcast Summary

    • Debating Precision and Controversial PracticesDoctors Brett Weinstein and Heather Heying discussed the importance of precision and accuracy in scientific research, butted heads over the necessity of determining exact winter percentage, and touched on controversial practices like WPATH files and amphibian milk. Sponsors included Van Man, MUD Water, and Helix.

During their livestream discussion, doctors Brett Weinstein and Heather Heying debated the precision of determining the percentage of winter that had passed. While Heying prided herself on accuracy and precision, Weinstein argued that good enough was good enough. They also discussed the WPATH files and the controversial practices surrounding them, as well as amphibian milk. Sponsors for the week included Van Man, MUD Water, and Helix, all of which the hosts endorse. Despite their differences in approach to precision, they found common ground and continued their engaging conversation.

    • Natural and Edible Skincare and Energy Boosting DrinkVan Man Company provides natural and edible skincare solutions with their tallow and honey balm, deodorant, and sunblock. MUDWTR offers an energy-boosting drink made with adaptogenic mushrooms and Ayurvedic herbs, along with organic and vegan non-dairy creamer and sweetener options.

Both Van Man Company and MUDWTR offer high-quality, edible and natural products. Van Man Company's tallow and honey balm, made with food-grade ingredients like bison tallow, Manuka honey, and organic royal jelly, is a versatile and effective skincare solution. Their deodorant and sunblock also feature edible ingredients and provide excellent results. MUDWTR, on the other hand, offers a delicious and energy-boosting alternative to coffee with their spicy and chocolaty drink, which contains adaptogenic mushrooms and Ayurvedic herbs. Their non-dairy creamer and sweetener are also organic and vegan, allowing for various options to suit individual preferences. Both companies prioritize the use of natural and organic ingredients, ensuring that consumers can trust the products they're using.

    • Helix Sleep: Personalized Mattresses for Optimal ComfortHelix Sleep offers personalized mattresses based on individual sleep preferences, with a quick quiz guiding users to their ideal mattress from 20 options. The mattresses provide excellent spine support and overall comfort with individually wrapped steel coils and premium foam layers.

Helix Sleep offers personalized mattresses based on individual sleep preferences, ensuring a comfortable and supportive night's rest. The Helix sleep quiz, which takes less than 2 minutes to complete, directs users to their ideal mattress from the brand's 20 options. Helix mattresses combine individually wrapped steel coils in the base with premium foam layers on top, providing excellent spine support and overall comfort. They offer a 100-night risk-free trial and 10 to 15-year warranties on all mattresses. Helix has received high accolades, including being ranked number 1 by GQ and Wired Magazine. The company also supports various groups with special discounts. On a different note, the WPATH files, leaked internal documents from the World Professional Association for Transgender Health, reveal that the organization's approach to transgender health care is not scientific or ethical. The files show that WPATH advocates for arbitrary medical practices, including hormonal and surgical experimentation on minors and vulnerable adults. The organization's members appear to be more engaged in political activism than science.

    • Prioritizing happiness in gender transition procedures for teenagers criticized as short-sightedHealthcare providers should focus on long-term consequences and full information for teenagers undergoing gender transition procedures, rather than prioritizing happiness as the primary goal.

During a discussion regarding informed consent for teenagers undergoing gender transition procedures, a recurring theme among healthcare providers was the prioritization of increasing happiness in these young people as the primary goal. This focus on happiness was criticized as a red flag, as it suggests a lack of understanding of human development and the complexities of adolescence. Happiness, being an ephemeral and instantaneous measure of satisfaction, is not an effective long-term goal. Instead, a more thoughtful and developmentally appropriate approach would be to consider the potential long-term consequences of these procedures and ensure that the teenagers are fully informed and prepared for the decisions they are making. Additionally, it was noted that some teenagers may change their minds about their gender identity over time, further emphasizing the importance of a thoughtful and informed approach.

    • Medicalization of young girls' discomfort with bodiesExcessive medical intervention for girls uncomfortable with their bodies during puberty can lead to mental health issues and harmful consequences, hindering their growth and self-discovery.

The medicalization of young people, particularly girls going through puberty, who express discomfort with their bodies and desire to halt menstruation, is a concerning trend. This practice, driven by a focus on happiness and instant comfort, can lead to unnatural reliance on adults and potentially harmful consequences. These clinicians, who may not be true experts, are encouraging children to view their bodies as misaligned with their minds, and offering solutions that can result in mental health issues and comorbidities. This is akin to the Milgram experiment, where individuals follow orders from someone in a lab coat, even when it leads to harm. It's crucial to remember that children are not always equipped to navigate complex adult issues, and excessive attention from adults can hinder their growth and self-discovery. Instead, we should foster an environment that encourages children to embrace their bodies and the natural process of growing up.

    • Autonomy in Gender Medicine: From Shield to SwordThe consumer-driven model in gender medicine raises concerns about unnecessary gatekeeping being removed and potential financial incentives, increasing the need for increased scrutiny and regulation.

The emphasis on autonomy in gender medicine has shifted from acting as a shield to protect patients from harmful treatments, to acting as a sword that allows doctors to provide any requested treatment as long as certain criteria are met. This consumer-driven model, as discussed in the WPATH files, has led to concerns about unnecessary gatekeeping being removed and potential financial incentives driving the industry. The Milgram experiment discussion highlighted the likelihood that many children may not question or resist treatments suggested by adults with advanced degrees, especially those with mental health issues. The ease of obtaining second letters for hormone treatments and surgeries, even from individuals with no genuine belief in the treatments, further highlights the potential for exploitation and the need for increased scrutiny and regulation in this field.

    • Complex ethical questions in gender-affirming healthcareSocietal norms and access to resources impact the interpretation of legality and patient autonomy in gender-affirming healthcare, raising concerns about potential consequences and the need for careful regulation.

The debate surrounding gender-affirming healthcare raises complex ethical questions, particularly when it comes to the use of experimental treatments and the potential for profit-driven decision-making. The case of interventions for gender dysphoria illustrates how societal norms and access to resources can impact the interpretation of legality and patient autonomy. For instance, during the COVID crisis, pharmacies blocked the fulfillment of prescriptions for ivermectin despite it being a legal prescription drug, while in contrast, there's a push to allow gender-affirming treatments regardless of cost. The ambiguous nature of these double standards raises concerns about the potential consequences of these interventions and the need for careful regulation. Additionally, the experimental use of hormones and surgeries to create bespoke gender identities challenges our understanding of what it means to be human and raises questions about the limits of medical intervention.

    • Hormonal treatments' complexitiesApproach hormonal treatments with caution and a deeper understanding of their complexities to avoid unintended consequences and irreversible changes

Hormones, such as puberty blockers and melatonin, are complex systems that cannot be simplified into a cause-and-effect relationship. Treating them as such can lead to unintended consequences, including dependency and disruptions to the body's natural development. The body's hormonal system is a dynamic and intricate process, and interfering with it can result in irreversible changes. For instance, blocking puberty can prevent the body from discovering how to function as an adult, and taking melatonin pills may not affect the subcellular melatonin that truly impacts mood, sleep, and antioxidant properties. Ultimately, we should approach hormonal treatments with caution and a deeper understanding of their complexities.

    • The Connection Between Our Bodies and the Natural WorldOur bodies are influenced by the natural world in various ways, including vitamin D and melatonin production, circadian rhythm regulation, and more. Our actions, like altering our environment, can have unintended consequences.

Our bodies and the natural world are intricately connected in ways that are still being discovered. Our relationship with the universe, specifically our star, influences various biological processes within us, including the production of vitamin D and melatonin, and the regulation of our circadian rhythms. This interplay is more complex than we can fully understand, and it's important to remember that our actions, such as altering our environment with indoor living or artificial light, can have unintended consequences. In the realm of healthcare, it's crucial to approach autonomy with caution and consider the potential harm. The concept of "cisnormativity," often used in transgender discussions, is a term that can be misleading as it does not mean normalcy but rather, the expectation that people identify and express their gender in certain ways. It's essential to be aware of these complexities and the potential pitfalls of making assumptions or taking actions without a full understanding of the consequences.

    • Manipulating normativity to erase straight as normalSocietal standards for evaluating behavior and outcomes (normativity) have been manipulated to erase the idea that being straight is normal, promoting gender transition as inevitable and necessary, while obscuring the need for open dialogue and support.

The concept of normativity, which refers to societal standards for evaluating behavior and outcomes, has been manipulated to erase the idea that being straight is normal. This is evident in the usage of terms like "heteronormative" and "gender doula," which have been appropriated to promote the idea that having a clear gender identity is not only important but necessary. The blurring of distinctions and the creation of impressions of inevitability serve to normalize gender transition and obscure the need for open dialogue and support for those struggling with their identity. It is essential to recognize the potential harm in these trends and advocate for a more nuanced and inclusive understanding of gender and sexuality.

    • Private vs Public Discussions of WPATH CliniciansWPATH clinicians privately acknowledge risks and unknowns of gender transition procedures but publicly claim zero regret rates and successful outcomes, raising ethical concerns and questioning informed consent for adults and children.

There is a significant disconnect between the private discussions and public declarations of WPATH clinicians regarding the risks and unknowns of gender transition procedures. While they acknowledge the pain and agony associated with these interventions internally, they publicly claim nearly zero rates of regret and successful outcomes. This raises serious ethical concerns and questions the validity of informed consent for both adults and children undergoing these procedures. The lack of transparency and independent testing is alarming, especially given the irreversible nature of some gender transition surgeries. It is crucial for there to be government entities and independent testing to ensure informed consent is upheld and patients are fully aware of the potential risks and consequences.

    • Principles of subsidiarity in pediatric transgender health careThe application of subsidiarity in pediatric transgender health care is controversial. While some argue for patient autonomy, critics argue that parents' involvement is necessary for informed consent and long-term consequences.

The application of the principle of subsidiarity in the context of pediatric transgender health care is a subject of controversy. The principle, which advocates for decisions to be made at the lowest effective level, has been invoked in a published paper arguing for the decisional authority to lie with the patient. However, critics argue that this is a violation of subsidiarity, as parents, who may have more knowledge and understanding, are being overruled. The discussion also highlighted the complexity of some cases, where young people are unsure about their gender identity and the potential long-term consequences of hormone blockers and surgeries. The lack of clear information and exploration of nonbinary options before irreversible interventions have raised concerns about informed consent. These issues underscore the need for a nuanced and thoughtful approach to decision-making in pediatric transgender health care.

    • The long-term effects of medical transitions for gender dysphoria are not fully understood. While medical transitions may bring initial relief, they can lead to further dysphoria and regret in the long term. Encouraging young people to explore the world and adapt to challenges can lead to personal growth and a better understanding of themselves.

The medical transition process for gender dysphoria, while bringing initial euphoria and relief, can lead to further dysphoria and regret in the long term. The honeymoon period of elation often lasts for a few years, but the realization of unchanged personal issues and unsolved problems can lead to disappointment and a sense of being trapped as a patient for life. The industry selling solutions to this discomfort exploits the clinical language of autonomy, but in reality, it makes individuals more subservient and less capable of solving their own problems. Instead, it's essential for individuals to face challenges in the world, learn to adapt, and discover their unique strengths and passions. The long-term effects of medical transitions are not fully understood, and short-term follow-up studies may not accurately represent the experience of many individuals. It's crucial to encourage young people to explore the world, find challenges, and learn to adapt and overcome obstacles, rather than relying on medical interventions as a quick fix.

    • Debating the complexities of treating gender dysphoria and comorbiditiesThe treatment of gender dysphoria and comorbidities, such as obesity and DID, is a subject of ongoing debates within certain communities. Some argue for affirmative care, while others emphasize health concerns. Providers must be knowledgeable and skilled to offer evidence-based care.

Growing up involves discovering one's strengths and weaknesses, and learning to live with them. However, there are ongoing debates within certain communities regarding the treatment of gender dysphoria and comorbidities, such as obesity and dissociative identity disorder (DID). Some argue that denying certain surgeries due to health concerns is a form of discrimination, while others emphasize the importance of considering the potential risks and complications. The WPATH community has discussed the complexities of obtaining informed consent for sex trait modification hormones and surgeries from individuals with plural identities, who may face challenges in reconciling their gender dysphoria and DID diagnoses. The affirmative care approach, which emphasizes validating and supporting individuals' identities, is being applied to various gender and plural identities, raising questions about its implications and potential limitations. Ultimately, it is crucial to ensure that medical and mental health providers are equipped with the necessary knowledge and skills to provide affirming and evidence-based care to individuals navigating these complex issues.

    • Approaching radical interventions with caution for childrenClinicians grapple with ethical concerns regarding body modifications, therapy for plural personalities, and gender confusion. Agreement among all personalities is necessary before proceeding, but the validity of these interventions is questionable due to the lack of clear rationality and articulation in the transcript.

The use of radical interventions, such as body modifications and therapy for plural personalities and gender confusion, should be approached with caution, especially for children. The idea of requiring external apps for communication between personalities and the potential presence of silent opposing personalities raises ethical concerns. The recent WPATH files reveal clinicians grappling with these complex issues, emphasizing the need for agreement among all personalities before proceeding. However, the lack of clear rationality and articulation in the transcript raises questions about the validity of these interventions. The NHS's recent ban on regular prescriptions of puberty blockers is a positive step, but the publication of an article advocating for these interventions in New York Magazine highlights the ongoing debate on the issue. Ultimately, it is essential to consider the potential long-term consequences and the inner workings of the individual's psyche before making such significant changes.

    • The Complexity of Transgender Rights DebateDespite societal beliefs, the freedom to alter one's sexual biology is a source of personal liberation for some, causing anxiety for those opposed. Nuanced and rational discourse is needed to protect all individuals involved.

The debate surrounding transgender rights and gender identity is complex and deeply rooted in societal beliefs and historical contexts. The hypothesis from the 1970s suggesting that the decline of traditional gender roles would lead to a decrease in people wanting to change their sex has been contradicted by recent trends. Instead, some individuals view the freedom to alter their sexual biology as a site of personal liberation. This idea is causing anxiety for those opposed to transgender rights, who fear the potential disruption to traditional gender and sex categories in society. The anti-trans movement's arguments often rely on misconceptions and sophistry, such as the claim that women are not truly women unless they are sufficiently vulnerable to rape. These arguments obscure the real issues, including the potential risks of medical interventions for transgender youth and the need to address historical vulnerabilities faced by women. Ultimately, the debate requires nuanced and rational discourse to ensure the protection and well-being of all individuals involved.

    • Cecilian Amphibians Provide Extensive Parental CareCecilian amphibians produce a milk-like secretion and offer nutritious skin for their offspring, highlighting the complexity and diversity of parental care in the natural world.

The Cecilian, a type of burrowing, limbless amphibian, provides extensive parental care through the production of a milk-like secretion and the offering of nutritious skin for their offspring. This third group of amphibians, which includes Sicilians, is often overlooked due to the more commonly known frogs and salamanders. The Cecilian's offspring emit high-pitched clicks near their mother's cloaca, which triggers the production of a high-carbohydrate, high-lipid secretion from the mother's oviducts. In some species, the babies also consume the lipid-rich skin of their mother as she is preparing to give birth or have her eggs hatch. The babies' vocalizations, which are likely not true vocalizations but mechanical noises, solicit this milk-like secretion from their mother. This discovery highlights the complexity and diversity of parental care in the natural world.

Anecdote in Science: The 224th Evolutionary Lens with Bret Weinstein and Heather Heying

In this 224th in a series of live discussions with Bret Weinstein and Heather Heying (both PhDs in Biology), we talk about the state of the world through an evolutionary lens.

In this episode, we discuss animal behavior: the implications of an orangutan using a medicinal plant to treat a wound, and the one-off videos that we’ve all seen—of cats preventing toddlers from falling, of a beluga whale retrieving a woman’s phone. What does this say about the minds of these other organisms, and about us? Also: dog domestication, from wolves, but not from foxes. And: the New York Times publishes more insipid garbage about vaccine injury, Bret returns to the question of what safety means, and The Nation turns its back on its muckraking roots.

Never Alone: The 223rd Evolutionary Lens with Bret Weinstein and Heather Heying

Never Alone: The 223rd Evolutionary Lens with Bret Weinstein and Heather Heying

In this 223rd in a series of live discussions with Bret Weinstein and Heather Heying (both PhDs in Biology), we talk about the state of the world through an evolutionary lens.

In this episode, we discuss science and religion, materialism and other ways of understanding our world. Can analysis and quantification explain everything? Can narrative, intuition, and creativity? All are or should be part of the scientific process; science is incomplete with only one mode. We also discuss models, how they fail, how difficult they are to assess, and what one recent climate model has to say about the role of air pollution in Earth’s temperature. Enlightenment values require that we use our freedom to inquire about all things that we are curious about. Also: happy Beltane!

Why Darwin was Right: The 222nd Evolutionary Lens with Bret Weinstein and Heather Heying

Why Darwin was Right: The 222nd Evolutionary Lens with Bret Weinstein and Heather Heying

In this 222nd in a series of live discussions with Bret Weinstein and Heather Heying (both PhDs in Biology), we talk about the state of the world through an evolutionary lens.

In this episode, we discuss evolution, and how we know that it is true. We discuss the different kinds of beliefs that people have—beliefs that attempt to reconcile with reality, vs beliefs that reconcile with social standing and comfort. If your beliefs put you on the outs with your friends, do you change your beliefs, or your friends? Are you willing to give up the cocktail party for reality? We also discuss research published in 2000 and funded by Fauci’s NIAID, which demonstrated that it was possible to make chimeric coronaviruses, swapping the spike protein from one to a different species entirely. Finally: sex-biased gene expression, in which genes are expressed differently in the two sexes, in anatomical systems as varied as brain, kidneys, and liver.

A conference on conscientious objection in medicine and the role of conscience in healthcare practitioners’ decision making, Oxford 2015. The conference aimed at analyzing from a philosophical, ethical and legal perspective the meaning and the role of “conscience” in the healthcare profession. Conscientious objection by health professionals has become one of the most pressing problems in healthcare ethics. Health professionals are often required to perform activities that conflict with their own moral or religious beliefs (for example abortion). Their refusal can make it difficult for patients to have access to services they have a right to and, more in general, can create conflicts in the doctor-patient relationship. The widening of the medical options available today or in the near future is likely to sharpen these conflicts. Experts in bioethics, philosophy, law and medicine explored possible solutions.

5 - Inside ESG: A sceptic vs a believer, our experts face off

The amount of money flowing into investment funds that claim to invest in sustainable businesses has surged in recent years. More companies than ever have embraced policies with environmental, social and corporate governance criteria as their central plank. But there's a debate over what ESG investing can achieve, particularly when it comes to cutting carbon emissions and addressing climate change.

In the final episode of our five-part series on ESG investing, we hear from two leading FT voices on opposite sides of the argument. Cheering the rise of ESG is Gillian Tett, the FT’s US editor-at-large and co-founder of Moral Money, the FT team that covers sustainable business and finance; and casting a sceptical eye is the FT’s US financial commentator Robert Armstrong, who also writes Unhedged, the FT newsletter on Wall Street.
